  • Helpful information but don’t appreciate the scare tactics.

    The info is generally good, but they all plug the Cleveland Clinic hard at the end of each podcast, and make it sound like you’re doomed if you don’t go to a cardiothoracic surgery “Center of Excellence” which is maybe not even a real thing — google it.

  • Pacemaker with ICD

    I have a Medtronic Pacemaker with ICD since January 2021. The Pacemaker is configured for biventricular pacing. It has Bluetooth and along with an App on my I-phone automatically sends reports to my electrophysiologist at a major hospital.

    I had A-Fib and Flutter that medicines weren’t able to control (even Amiodarone).

    Back in October 2001, I had a St. Jude mechanical aortic and mitral valve implanted (due to both leaking). The mechanical valves have worked out very well over the 23 years.

Q: A very good question. I think the mitral valve leaks for different reasons. So if the leak is related to the fact that the valve is prolapsing, or the part of the valve is such that it is broken the cords, then it is best to repair the valve if the repair is possible.
Repair is preferred when feasible, especially if there is a structural issue with the valve; it can provide better long-term durability and outcomes.

Q: What are the initial tests and most common treatments for acute pericarditis, and when might MRI be used?
Initial testing includes ECG, echocardiography, and blood work (CRP among others). First-line therapy usually involves NSAIDs like ibuprofen or aspirin and colchicine, with steroids less preferred nowadays. MRI becomes particularly helpful when there are high-risk features, uncertain diagnosis, or recurrent/persistent symptoms to gauge inflammation and guide escalation of therapy.

Pericarditis is inflammation of the lining around the heart. It often presents with sharp chest pain and distinct symptoms from a heart attack, which is due to reduced blood supply to the heart muscle. It can be caused by viral infections, autoimmune processes, or post-procedural injury, and diagnosis typically starts with ECG, echo, and blood work.

We ensure close monitoring post-operation with iterative echocardiograms and CT scans to check the repair's effectiveness and any signs of complications.

Q: What are the benefits of having a valve re-implantation versus having a biological or mechanical valve?
Valve re-implantation can save younger patients from lifelong anticoagulation and has shown durable results; many patients live without complications or need for replacement.
