  • The ads make no sense

The content is excellent! I would appreciate a little more humor but that’s just a personal preference about podcasts.

The main issue is the ads. Way too often ads are being placed in weird spots (even mid sentence at times) rather than at natural transition points. The content of the ads also makes little sense. This is a podcast for healthcare professionals and many times the ads seem way out of touch with the audience. For example, there were many ads encouraging the audience to apply for tr... more

DEI 24/7. Equity does not equal equality. It’s not possible to get equity without constant taking from someone to give to someone else, the true definition of a communist state. It won’t take long for those that are the most gifted and talented to be taught, why try this hard, when merit is no longer the standard

  • Please bring back Dr Chutka

The pod has taken a sharp turn from hard medical topics that are based on data and patient care to social justice. It’s quite disheartening that the new episodes hinge on not a shred of evidence but equity opinions. I have to now vet episodes to know if I am going to learn something or be lectured by a social justice advocate based on politicized topics. Please pay attention to the host when creating podcasts.

Talking Points

Recent interactions between the hosts and their guests.

Hantavirus
Q: How should suspected hantavirus cases be managed in terms of hospitalization and monitoring?
Management depends on clinical status and exposure; stable patients may be observed at home with clear instructions to seek care for worsening symptoms, while those with hypoxia or pulmonary involvement should be hospitalized.
Hantavirus
Q: What are the incubation periods and early signs that should raise suspicion for hantavirus?
Incubation typically two to four weeks but can range from one to eight weeks; early symptoms are nonspecific viral features like fever and myalgias, followed by potential pulmonary symptoms if the disease progresses.
Hantavirus
Q: Is hantavirus in the US, and what does that imply for clinicians and the public?
There are US strains like Sin Nombre that cause pulmonary disease but human-to-human transmission is extremely unlikely; awareness of exposure risk and regional patterns helps guide suspicion and testing.
Fibromyalgia
Q: What does a typical treatment plan look like for a patient with established fibromyalgia?
A multimodal, multifactorial approach forms the foundation, likened to a salad of strategies including non-pharmacologic therapies (education, exercise, sleep hygiene, stress management) and medications (often duloxetine or pregabalin) tailored to symptom predominance, with gradual tapering if opioids are involved.
Fibromyalgia
Q: How is fibromyalgia diagnosed given the lack of a definitive test?
Diagnosis rests on clinical symptoms and criteria, with historically used tender-point examinations now largely superseded by broader symptom-based criteria; there is no smoking gun test, and physicians rely on patient history and symptom patterns.
