Nicely done, succinct, and enjoyable podcast. Recording quality could be better.

  • Not sure who this podcast is for

The analysis of the papers is pretty unsophisticated, e.g., slipping into causal language with studies not designed to determine causality. The practice advice is pretty general conventional wisdom stuff. Despite being short, still not worth the time.

  • Great but interviews can be irksome

Great short content that’s applicable to daily practice. Lately, one NP has massive vocal fry and the other stutters and stops her sentences so frequently that it can impede listening.

Talking Points

Recent interactions between the hosts and their guests.

Pulse Oximetry—It’s Not Always Right - Frankly Speaking Ep 491
Q: What are the practical implications for a patient like John with COPD and a borderline pulse ox reading?
In cases like John's, it's important to question the absolute reliability of the pulse ox given his risk factors and consider additional assessment such as arterial blood gas or ED evaluation to rule out true hypoxemia and guide timely treatment.
Pulse Oximetry—It’s Not Always Right - Frankly Speaking Ep 491
Q: How exactly does a pulse ox work, and what are the factors that can cause it to be inaccurate?
A pulse oximeter uses light absorption at two wavelengths to estimate the percentage of hemoglobin that is oxygenated; various factors such as abnormal hemoglobins, smoking, patient movement, severe anemia, and darker skin tones can distort readings, with darker skin tones often causing systematic overestimation of true oxygenation.
What Are We Missing? Recognizing Symptoms of Maternal Stroke - Frankly Speaking Ep 490
Q: What are the common signs and symptoms of maternal stroke? And how do we manage them?
The discussion outlines focal neurological signs such as weakness or sensory changes, speech or vision issues, and also non-focal symptoms like severe headaches; in postpartum patients these symptoms may be mistaken for preeclampsia, so clinicians should maintain a high index of suspicion and pursue imaging when red flags are present.
What Are We Missing? Recognizing Symptoms of Maternal Stroke - Frankly Speaking Ep 490
Q: We have to think about what could be causing this and included in our differential is postpartum maternal stroke. Can you talk about the common risk factors for this?
Dr. Feeney explains that postpartum women are often hypercoagulable and that existing comorbidities like hypertension, obesity, diabetes, and a history of coagulation disorders heighten the risk, with preeclampsia being a particularly significant factor. The risk is highest in the first two weeks postpartum.
ADHD Medications Don’t Work the Way We Thought - Frankly Speaking Ep 488
Q: How did they do the functional MRI study?
They used data from the ABCD study, scanned thousands of children over five years, and compared stimulant users, non-users with ADHD, and controls to identify how brain networks differed with stimulant use, including sleep as a variable.
