  • Retirement rant is ridiculous

The comments on not retiring are criminally insensitive. Many people have no choice about retirement age. Many jobs are unrewarding and stressful, not rewarding or providing purpose. By contrast many retirement activities are engaging and meaningful. Clearly it has to be approached carefully but it can be fulfilling and health promoting.

  • Big fan!

I love this podcast! I’m a first year medical student and listen to it as productive procrastination (or when I’m too deep in the biochem/anatomy study weeds and need a reminder of what I’m moving towards). I attribute at least 85% of the smart things I say during class to this show. Plus it’s fun!

Talking Points

Recent interactions between the hosts and their guests.

#529 A Case of Syncope with the Clinical Problem Solvers
Q: Where do you go next after left heart catheterization and ICD placement?
The probability of ischemia is reduced due to lack of epicardial disease, but in a patient with sarcoidosis, you must pursue imaging of the myocardium (cardiac MRI with gadolinium and FDG-PET) to evaluate for inflammatory involvement and guiding therapy.
#522: Healthy Aging with Dr. Elizabeth Eckstrom
Q: What role does Tai Chi play in reducing fall risk and improving cognition?
Tai Chi provides dual-task training combining movement and cognitive engagement, associated with substantial fall risk reduction and cognitive benefits, often more effective than some conventional therapies for older adults.
#522: Healthy Aging with Dr. Elizabeth Eckstrom
Q: How do you address diet and protein for an older adult aiming to age healthily?
Recommend a Mediterranean-style plan with five colors of produce daily, emphasize mostly plant-based protein (about 70%), use low-fat dairy and fish, limit red meat, and consider practical protein sources like beans, lentils, nuts, and smoothies if needed.
#522: Healthy Aging with Dr. Elizabeth Eckstrom
Q: Where would you start with Ms. R, who is retiring and looking to stay active and engaged?
Begin with a comprehensive plan focusing on aerobic activity, strength training, social engagement, and enjoyable activities like Tai Chi, while addressing diet and potential mobility limitations, and tailoring goals to what matters most to her (e.g., grandchildren and community involvement).
#519 Mild Autonomous Cortisol Secretion
Q: What exactly is mild autonomous cortisol secretion and how does it differ from traditional Cushing syndrome?
MACS is cortisol production from an adrenal nodule independent of pituitary ACTH stimulation. Unlike overt Cushing syndrome, many patients lack classic stigmata, though the distinction can be blurry in modern practice due to changes in obesity management and diagnostic thresholds; the key is abnormal dexamethasone suppression testing and evidence of cortisol production despite normal ACTH physiology.
