  • Fantastic Resource

Last fall I started working at a hospital in a support role with very little healthcare experience. Soon after I started listening to Turn on the Lights to help me understand more of my new environment. I cannot tell you how much this has helped me with background contextual understanding that my colleagues take for granted. I also feel more up to date on current healthcare challenges and opportunities which allows me to contribute more in discussions surrounding planning and policy.

Talking Points

Recent interactions between the hosts and their guests.

Who Sets the Table for Quality Measurement in U.S. Health Care? with Brenna Rabel & Michelle Schreiber
Q: What is the future of value-based programs like MIPS and how are they evolving?
MIPS is moving toward Value Pathways to align measures with specific specialties, reduce redundancy, and pair quality metrics with cost and improvement activities, making the program more meaningful for clinicians and patients alike.
Who Sets the Table for Quality Measurement in U.S. Health Care? with Brenna Rabel & Michelle Schreiber
Q: Why are some measures controversial or heated, and can you give an example?
Controversies arise when measures reflect not just clinical best practice but also political, financial, or industry-driven agendas; for example, the A1C cutoff for diabetes control has been debated because all-cause optimal control may vary by patient population, and setting the cutoff can influence how providers are judged on tests rather than patient outcomes.
Who Sets the Table for Quality Measurement in U.S. Health Care? with Brenna Rabel & Michelle Schreiber
Q: Who gets to set the table for quality measurement and who is involved in choosing which measures are used?
A broad coalition including CMS, private payers, academics, clinicians, patients, and policymakers contribute to measure development, with CMS often taking a leading role, though others shape the agenda and provide cues.
AI, Interoperability, and the Next Era of Quality Measurement with Jeff Geppert
Q: What is a consensus-based entity, and how does it fit into the modern ecosystem of hundreds or thousands of quality measures?
He describes the two main paths—endorsement and pre-rulemaking review—and how input from clinicians, data experts, IT professionals, and patients helps evaluate whether a measure should be used for accountability or public reporting.
Disrupting the Aging Services Model Through Community-Based Care with Marta Corvêlo
Q: What drives the shrinking caregiver ratio in the US?
The changes in family dynamics, with fewer family members available to provide care due to various reasons, including geographic dispersion and increasing demands on family caregivers themselves.
