I’ve binge listened to the entire back catalogue of podcasts. It’s been very comforting to hear about the IVF journey from someone so experienced. I feel much more prepared going into our first consultant appointment in a few weeks. Thank you Prof!

  • Patriarchal

I did like this podcast until Prof gave the whole “just relax and it’ll happen” spiel. Oh yes, the hysterical woman just needs to take an Ativan and relax and she too will fall pregnant. That’s why refugees and prisoners of war and rape victims fall pregnant. Because of their low levels of cortisol. 🙄🙄🙄🙄🙄🙄🙄🙄🙄🙄🙄🙄🙄🙄🙄

No! If you’re having trouble getting pregnant, seek help! There could be something wrong and you’ll waste precious reproductive years “just trying to relax”.

Talking Points

Recent interactions between the hosts and their guests.

550. Can a Different IVF Protocol Improve Your Chances of Success
Q: Prof, you talked about male infertility before. How do you involve the male partner meaningfully in the IVF process beyond the initial semen analysis?
The male partner is engaged through emotional support and practical steps, including ensuring optimal sperm quality around the time of egg collection; typically advice centers on timing, health optimization, and collaboration with the partner to maximize the chances of a successful cycle.
547. Frozen Embryos and PGT: Understanding the Risks and Benefits
Q: Would you unfreeze embryos to PGT test them?
Unfreezing and re-freezing for PGT carries a small risk of losing embryos, so the decision depends on the number of embryos available and the patient's age; PGT can significantly improve the odds if there are multiple embryos, particularly for women over forty.
548. IVF Ethics: What Really Worries Fertility Experts Today
Q: As fertility technology advances, what ethical challenges worry you the most?
The speaker recalls IVF's origins as experimental and emphasizes the need to monitor long-term outcomes, while recognizing that public and religious concerns persist; he stresses continued vigilance about safety and ethics as technology evolves.
551. The Truth About Ovarian Tissue Freezing and IVF Pregnancy Monitoring
Q: Katie asks about a positive beta at 10 days post-transfer with HCG 288 and progesterone 111, and whether a repeat beta is warranted or if there is cause for worry.
The host responds that a beta of 288 at day 10 is a strong indicator of pregnancy and suggests repeating the HCG in a couple of days to confirm a rising trajectory, which is expected to double and guide further monitoring, while progesterone may be less critical if the pregnancy is viable. Overall, the guidance is to verify a rising trend to establish viability and plan the next steps.
549. Beyond Success Rates: How to Find a High-Quality IVF Clinic
Q: We have a question from May. She asks for success rates in IUI and IVF in a 37-year-old.
The expert explains that IUI in a 37-year-old yields around 15-18% per cycle in early attempts, dropping significantly after a couple of cycles, and that IVF offers roughly 30-35% per embryo transfer at that age, with cumulative rates improving after multiple embryo transfers. He also notes that if there are underlying issues, IUI may be no better than background rates, and emphasizes considering overall strategy, including potential multiple embryos and cumulative outcomes.
