I’ve binge listened to the entire back catalogue of podcasts. It’s been very comforting to hear about the IVF journey from someone so experienced. I feel much more prepared going into our first consultant appointment in a few weeks. Thank you Prof!

  • Patriarchal

I did like this podcast until Prof gave the whole “just relax and it’ll happen” spiel. Oh yes, the hysterical woman just needs to take an Ativan and relax and she too will fall pregnant. That’s why refugees and prisoners of war and rape victims fall pregnant. Because of their low levels of cortisol. 🙄🙄🙄🙄🙄🙄🙄🙄🙄🙄🙄🙄🙄🙄🙄

No! If you’re having trouble getting pregnant, seek help! There could be something wrong and you’ll waste precious reproductive years “just trying to relax”.

Talking Points

Recent interactions between the hosts and their guests.

543. What Fertility Specialist Look After a Failed IVF Attempt
Q: Prof, have you had any patients come to you with asking for a second opinion after a bad experience in another clinic?
Yes. Patients often seek second opinions because they want to understand what went wrong, what could be done differently, and to regain trust by having a clear, detailed review of prior treatments and possible alternatives.
538. Starting IVF Right: Tests, Mosaic Embryos, & Transfer Choices Explained
Q: In which clinical scenarios do you still favor fresh cycles, and when is a frozen cycle clearly superior?
Frozen cycles are not universally superior; they are advantageous mainly when used for genetic testing, while overall pregnancy rates are similar between fresh and frozen transfers; the choice depends on patient-specific factors and testing needs.
538. Starting IVF Right: Tests, Mosaic Embryos, & Transfer Choices Explained
Q: So, Rolf, how do you interpret mosaic embryo results in real world practice, and how do you guide patients through the uncertainty of transferring one?
Mosaic embryos are treated as untested embryos for counseling purposes; pregnancy and miscarriage rates align with untested embryos, and most mosaics end up being fine unless specific trisomy 21-like mosaics are involved; counseling emphasizes that mosaicism does not inherently doom a pregnancy, and decisions are tailored to each case.
537. Repeated IVF Transfer Failures: What to Test Before Your Next FET
Q: Two FETs included immune protocol, steroids, Clexane. What kind of further testing should I be asking for? Or do you think it's a numbers game?
The answer emphasizes not giving up on testing and tailoring the plan to the patient, including ensuring the physician has CREI qualifications, considering embryo genetic testing to avoid aneuploid transfers, and optimizing progesterone and transfer conditions; overall, it's about moving from trial-and-error to a targeted, evidence-based approach with a specialist.
535. Recurrent Miscarriage Explained: Which Tests Matter and What Actually Helps
Q: For recurrent miscarriage, which baseline tests are worth doing and which results actually change treatment?
Baseline testing should focus on ensuring there is no chromosomal issue like balanced translocation, evaluating the uterus for structural problems, and considering antiphospholipid syndrome and thrombophilia—tests with a clear diagnostic and therapeutic pathway. Immunological tests beyond evidence-based ones are generally not helpful, and if an test result would not change management, it may be prudent to avoid it.
