  • Great Resource!!

ACDIS Radio is a great place for CDI, Coding, HIM & even Providers to come for industry updates and what’s happening in the CDI industry. I’m grateful that it’s now available via podcast! I was accustomed to missing the live shows due to a hectic schedule and would listen to archived shows. Now I can listen to the show via podcast during my commute to & from work. Great job ACDIS!

CDI teams need to collaborate with physician advisors to educate on updating problem lists accordingly to reflect current diagnoses and treatment.
Problem list sequencing
Q: What are the most common problems you encounter and how do you address them?
Common problems include unupdated lists that fail to reflect current patient conditions, leading to inaccurate coding and patient care.
Problem list sequencing
Q: Can you use the problem list to capture diagnoses?
Some coders say no, while others capture valid diagnoses that are being treated, regardless of their listing on the problem list.
Problem list sequencing
Q: Could you please tell me a little bit about the problem list and explain also how our problems are sequenced?
The problem list in the inpatient setting can include diagnoses from previous visits and may be listed randomly rather than by order of significance.
Malnutrition quality improvement: A case study
Q: Are there any compliance concerns related to dieticians adding malnutrition to the problem list?
Dieticians add the malnutrition diagnosis to the problem list, but the responsibility for documentation accuracy lies with the providers who must address it in their notes.
