    For those of us in the field of pediatric cardiology, this is a great addition to our ways to learn and stay current. As all things new, it has some pros and cons.

    Pros: Covers relevant topics. It also covers topics from the full range of pediatric cardiology. You can tell Dr. Pass is listening to feedback consistently. Mavacamten is a cardiac myosin inhibitor that reduces hypercontractility by limiting the number of active myosin heads, shifting the heart toward a more relaxed state. This can relieve LV outflow tract obstruction, improve diastolic function, and potentially reduce pathological remodeling. In adults, it reduced gradients and improved symptoms, which provided a strong rationale to test it in pediatric patients with obstructive HCM.

Is there any evidence in the literature for this sort of a question?
The evidence is not strong enough to support routine empiric pacing for all such high-risk cases; however, the data suggest you could consider temporary pacing in particularly complex scenarios while monitoring recovery, rather than preemptively placing a permanent device unless pacing is clearly indicated by symptoms or sustained dysfunction.

Q: Great to have you both. I was wondering, Audrey, oftentimes when I do these interviews, the first thing I'll often ask the guest is maybe if you as the author could summarize for the audience what you think are maybe the four or five most important takeaways from your study of sinus node dysfunction after congenital heart surgery.
The 17 percent incidence shows sinus node dysfunction is a frequent post-operative issue, but recovery is common and often rapid, with most patients regaining normal function within a day or two; only a small fraction requires permanent pacing during the initial hospital encounter; certain surgeries and patient anatomies carry higher risk; and pacing decisions should be guided by symptoms and recovery trajectory rather than the dysfunction alone.
