Talking Points

Recent interactions between the hosts and their guests.

Pediheart Podcast #376: What Are The Critical Components Of A Congenital CCT Program?
Q: What are the financial and reimbursement challenges facing congenital CT programs, and how are they being addressed?
There's a gap between the clinical value of cross-sectional imaging and reimbursement; programs are advised to quantify productivity across modalities and push for reimbursement standards that reflect the workload and responsibilities involved in managing multi-modality imaging.
Pediheart Podcast #376: What Are The Critical Components Of A Congenital CCT Program?
Q: Why did the authors feel a paper outlining basic resources for a congenital CT program was essential now?
The field has seen exponential adoption of CT in congenital heart disease, outpacing available resources, and there was a need for a common, authoritative set of standards to guide institutions in building or upgrading programs.
Pediheart Podcast #374: Tricuspid Regurgitation And Outcomes In The HLHS Patient
Q: What are the three most important takeaways from the SVR trial regarding TR in single-ventricle patients?
The SVR trial shows that >mild TR at baseline and early timepoints is linked to worse transplant-free survival; TR tends to worsen after Norwood but stabilizes with age; and repairs at Norwood/Stage 2 did not improve survival, whereas Fontan-era repairs showed better early outcomes, though causality remains uncertain due to confounding by indication and limited follow-up.
Pediheart Podcast #372: 30+ Year Follow-Up After The Arterial Switch Operation - The Melbourne Experience
Q: Can interventions usually happen via repair or replacement when patients need aortic valve or root interventions?
Most of those valves are replaced, but I feel strongly that they must be repaired as they are structurally normal, and with proper techniques, durability can be achieved.
Pediheart Podcast #372: 30+ Year Follow-Up After The Arterial Switch Operation - The Melbourne Experience
Q: What are the major technical reasons that you think that patients with transposition and a VSD undergoing an arterial switch have higher rates of both right and left-sided re-intervention following surgery?
Patients with VSD operate later, have larger pulmonary arching leading to larger neo-aortic roots, and smaller aorta which can lead to higher intervention rates.
