Pediheart Podcast #383: Using Virtual Reality And 3D Modelling For Planning Complex Congenital Heart Surgery
Q: Could you explain the general workflow and why three different printed models (F1, F2, F3) are made?
The three models reflect a progressive workflow: F1 is a rigid 3D-printed template; F2 is a digitally flattened version that is easier to trace on pericardium; and F3 is a paper 1:1 template for quick, low-cost intraoperative guidance, enabling accurate patch shaping and anchoring.
Pediheart Podcast #382: Outcomes Of Mavacamten In Adolescent Patients With HCM
Q: Could you summarize how mavacamten is believed to work and why it was thought useful for hypertrophic cardiomyopathy in children?
Mavacamten is a cardiac myosin inhibitor that reduces hypercontractility by limiting the number of active myosin heads, shifting the heart toward a more relaxed state. This can relieve LV outflow tract obstruction, improve diastolic function, and potentially reduce pathological remodeling. In adults, it reduced gradients and improved symptoms, which provided a strong rationale to test it in pediatric patients with obstructive HCM.
Pediheart Podcast Replay #322: Hepatocellular Carcinoma And The Fontan Operation
Q: Is there evidence that early identification of hepatocellular carcinoma leads to better outcomes?
Although unpublished in this episode's discussion, there is a strong trend toward improved survival when HTC is identified at earlier stages, with a potential benefit seen when patients receive curative therapies or are under active surveillance.
Pediheart Podcast Replay #322: Hepatocellular Carcinoma And The Fontan Operation
Q: What should the ACHD specialist do to surveil Fontan patients to reduce risk or catch hepatocellular carcinoma early?
Focus on comprehensive surveillance in adulthood, including baseline abdominal imaging with MRI (or CT if MRI is contraindicated) and follow with liver ultrasound every six months; also consider alpha-fetoprotein testing as part of a broader assessment, while recognizing that non-invasive fibrosis scores may have limited predictive value in this population.
Pediheart Podcast #378: Sinus Node Dysfunction Following Congenital Heart Surgery
Q: For example, if you had a heterotaxy single-ventricle patient who had AV valve regurgitation, who underwent repair of the AV valve, and maybe a Fontan at the same time, would that be someone that you might consider empiric pacing in given what you've found? Is there any evidence in the literature for this sort of a question?
The evidence is not strong enough to support routine empiric pacing for all such high-risk cases; however, the data suggest you could consider temporary pacing in particularly complex scenarios while monitoring recovery, rather than preemptively placing a permanent device unless pacing is clearly indicated by symptoms or sustained dysfunction.
