Q: Can you briefly outline the study design for our listeners as well as explain which patients were recruited, who was excluded, and how many facilities were included?
It was a retrospective cohort analysis of consecutive hip fracture patients over two one-year periods at a single site (St Vincent's Hospital in Sydney). The first period covered February 2021 to January 2022, and the second August 2022 to July 2023, with the analgesic care bundle implemented between the periods. Exclusions included patients under 50, multi-trauma cases, or those with injuries affecting mobilization or pain assessment.

A screener identifies eligible patients on wards, they may meet the patient and family on the ward, and a clinician visits soon after discharge to check setup, meds, and initial rehab plans, followed by home visits that progressively support mobility and independence.

AWHI expands a long-standing nursing-led discharge service, now comprising nurses, physios, OTs, social workers, and other allied health staff. The goal is to keep patients out of hospital longer when safe, prevent readmissions, and use the home environment to support rehab while maintaining hospital flow.
