Urine albumin-creatinine ratio (ACR) is more specific for glomerular disease and is easy to obtain; protein-to-creatinine ratio (PCR) includes all proteins and helps detect conditions like multiple myeloma. Often both are ordered initially to compare, especially in undifferentiated CKD.

Q: How do you interpret an estimated GFR with an aging population, and are there tables or guidelines to refer to for age-appropriate norms?
GFR typically declines with age; for example, a 60- to 70-year-old might have a GFR around 60–70, and people 80+ may have a GFR around 60 or higher as a normal range for age. If the GFR is below 60, evaluate for non-age-related causes.
