Thank you for producing this episode. Highly Recommended for all of us practitioners who deal with patients everyday.

I have a conscientious objection to this law, UNLESS the eligibility criteria will include Maximum palliative care input before a patient gets approval. If exceptional palliative care provision happens, then no one will actually need to use this law.

Talking Points

Recent interactions between the hosts and their guests.

Can the NHS win GPs over on advice and guidance, plus GP partnership fears
Q: What does NHS England's letter mean for how psychiatrists and other specialists might use referrals through SPOA in practice?
The letter emphasizes local co-design with GPs, keeping GP referral rights, and establishing clear response times; it also highlights that SPOA should not create a blanket rejection of referrals but should be used to streamline appropriate cases.
Can the NHS win GPs over on advice and guidance, plus GP partnership fears
Q: First up, let's talk about advice and guidance. Nick, this was one of the really controversial parts of this year's GP contract, wasn't it?
Yes, the discussion centers on shifting funding from an advice-and-guidance DES into core funding and removing a hard national target for diverting referrals; the aim is to reassure GPs that their right to refer remains intact while pushing for local, co-designed pathways.
What happens now GPs have rejected the contract, plus will GP pay rise by 3.5%?
Q: What does the 3.5% pay uplift mean for practices and staff?
The DDRB recommendation of a 3.5% uplift, accepted by the government, is an increase over the initial 2.5% uplift and requires practices to find funding to pass this on to all staff, while also navigating rising costs and minimum wage increases that may outpace the uplift.
What happens now GPs have rejected the contract, plus will GP pay rise by 3.5%?
Q: So Nick, first up, let's talk about the BMA referendum on this year's GP contract deal. What were the results?
The referendum results were decisive, with 98.9% of participating GPs and GP registrars voting to reject the 2026-27 contract, indicating a strong demand for movement from the government, though the government portrayed the result as not representative of opinion across the entire profession.
Dr Katie Bramall on why the new GP contract is unsafe and how GPs can fight back
Q: So why is it a bad deal for General Practice in your view?
The contract imposes unsafe changes that could cripple delivery, undermines the ability to refer and manage urgent and non-urgent care, and shifts risk and workload onto GP practices rather than supporting them with adequate funding and safeguards.
