  • Physician Associate episodes

I found this podcast as a means of learning about the Physician Associate role in the U.K. I am a Physician Assistant in the US, with hopes of moving to the U.K. While I haven't listened to all of the episodes, I found the episodes #40, 43, and 52 regarding Physician Associates to be extremely valuable. I appreciate the information given, as well as the interview with GP Joanna Munden regarding her PA and how the PA functions.

Thank you, Ben, for a wonderful podcast! I will continue listening... more

Talking Points

Recent interactions between the hosts and their guests.

Podcast - Matt Jones - The Carr-Hill Formula: Why Some Practices Are Missing Out on Hundreds of Thousands of Pounds
Q: How might reforms affect access to care and practice viability if implemented in Wales?
Reforms aimed at equalizing funding would prevent some practices from being overburdened while ensuring underfunded ones can maintain staffing and services, reducing burnouts and safeguarding patient access while acknowledging regional demographic shifts.
Podcast - Matt Jones - The Carr-Hill Formula: Why Some Practices Are Missing Out on Hundreds of Thousands of Pounds
Q: What reforms does Matt Jones propose to address the most glaring issues with Carr-Hill?
Proposals include introducing a floor to key indices, separating clinical workload from unavoidable costs, and possibly removing uplifts (like DDRB) from Carr-Hill to fund fixed costs directly, thereby reducing volatility and improving transparency.
Podcast - Matt Jones - The Carr-Hill Formula: Why Some Practices Are Missing Out on Hundreds of Thousands of Pounds
Q: Why are fixed costs not adequately considered in Carr-Hill, and what would change if they were treated differently?
Fixed costs like premises and admin overhead are not tied to the indices, so even with similar patient loads, a practice with higher fixed costs ends up underfunded; treating fixed costs separately would stabilize discretionary capacity and make funding more predictable.
Podcast - Matt Jones - The Carr-Hill Formula: Why Some Practices Are Missing Out on Hundreds of Thousands of Pounds
Q: What is the core mechanism that makes Carr-Hill so influential in funding differences between practices?
The formula uses multiplicative interactions of key indices, so small deviations in several areas compound to create large differences in total funding; this is why some practices miss out by hundreds of thousands of pounds.
Podcast – Sian Stanley – Can General Practice Lead System Transformation?
Q: What are the main challenges in aligning GPs, federations, and acute trusts?
Key challenges include maintaining GP buy-in, ensuring robust governance, balancing clinical autonomy with business discipline, and preventing the dilution of primary care leadership by hospital-centric priorities.
