  • Trauma cast

As a new trauma and acute care surgery APP, I love this podcast! It breaks down case base scenarios and logically discusses the course of care. So helpful and gives me good points to then go do my own reading on topics. Thank you!!

  • In the arena

In The Arena is (imho) the best medical podcast out there today.

It feels like an NPR podcast with a unique narrative style that keeps the listener engaged and the dialogue between the hosts sounds like they are genuinely enjoying themselves.

The topics are broad from clinical to professional and I find myself learning a lot with each episode.

Lastly, Shyam Murali has the perfect personality for this. He always seems knowledgeable on the topic but humble enough to ask the questions that al... more

  • Audio Quality

You don’t need to get anything fancy, but at least make sure there isn’t any background noise. Hearing phones, doors, and such is rather distracting and unprofessional.

  • Mostly great content, mediocre production value

The intro says it all, this is brass tacks, no frills presentation of expert insight and (to a lesser extent) relevant literature. And the audio quality is terrible. I'm not saying you need to shamble into a studio and lay down five takes, but it's 2018. There are reasonable recording technologies that don't sound like phone calls from my Nokia circa 1998. At four different volume levels.

Talking Points

Recent interactions between the hosts and their guests.

39th EAST Annual Scientific Assembly Compilation
Q: What were the main findings about ATV legislation impact on crashes?
Post-legislation data showed overall more crashes, especially on newly legalized road types, with evening-time crashes increasing and hospitalization costs rising for certain groups, while ED and Level I trauma center visits remained similar.
EAST/AAST/AAST-AMC: SCC Review Series - Nutrition and Endocrine
Q: When managing hyperglycemia in a critically ill patient, what target range and route of insulin administration are recommended?
Aim for a target glucose range of 140-200 mg/dL, use a continuous IV insulin infusion for most critically ill patients, and employ a facility-specific low-risk protocol with frequent monitoring to minimize hypoglycemia risk.
EAST/AAST/AAST-AMC: SCC Review Series - Nutrition and Endocrine
Q: How should we interpret gastric residual volumes in the context of enteral feeding for a critically ill patient?
Gastric residual volumes should not be the sole determinant of feeding cessation; assess clinically for distension, vomiting, bowel sounds, and signs of intolerance, and consider prokinetic therapy or post-pyloric feeding if high residuals persist.
EAST/AAST/AAST-AMC: SCC Review Series - Nutrition and Endocrine
Q: What is the recommended initial approach to feeding a hemodynamically unstable patient starting from a low trophic rate?
Initiate low-rate trophic feeds via the existing NG tube to establish gut flow and tolerance while prioritizing safety over rapid caloric delivery; escalate only as tolerance and perfusion improve, and monitor clinically for signs of intolerance.
EAST/AAST/AAST-AMC: SCC Review Series - Airway Management
Q: What precautions do you take prior to extubating a patient with a known difficulty airway in the ICU?
We perform a trial of spontaneous breathing, assess for adequate oxygenation and ventilation, ensure there is a cuff leak and have anesthesia at the bedside with a difficult airway kit.
