Q: What is your approach to managing these patients acutely and when do you decide to proceed to surgery?
Begin with medical management to lower IOP and suppress inflammation, using topical and possibly oral agents, and monitor corneal clarity and pupil status. Once the eye is medically stabilized and the view improves, reassess for surgical extraction of the cataract to resolve the underlying stimulus, typically within one to two weeks, while ensuring there is a clear plan to avoid synechiae and other complications.

I'd first assess the contralateral eye to gauge whether there is a lens-related mechanism versus primary angle-closure, then review the maturity of the cataract, check for signs of pigment dispersion or inflammation, and consider imaging like UBM or AS-OCT to evaluate the anterior segment. It's important to identify whether there is a pupillary block, a phacogenic process, or a pure open-angle mechanism, which will drive whether to pursue medical management first or move toward surgical removal of the lens.

Q: What systemic tests or imaging would you like to order for these patients?
Test for TB and syphilis; consider sarcoidosis workup (ACE, lysozyme, chest X-ray); evaluate for neoplastic mimics and perform extensive imaging as guided by clinical suspicion.
