Recent interactions between the hosts and their guests.

40. Choosing a Glaucoma Drop (Dr. Irfan Kherani)
Q: What is your approach to dual agents in glaucoma therapy?
There is a role for dual agents, especially for patients needing significant pressure lowering, but starting on separate agents is often more effective and preferable than combined medications.
40. Choosing a Glaucoma Drop (Dr. Irfan Kherani)
Q: How do you decide between alpha agonists and carbonic anhydrase inhibitors?
Alpha agonists may be slightly more effective, but their side effect profile is worse. We have to balance stinging from carbonic anhydrase inhibitors with the potential discomfort from alpha agonists.
40. Choosing a Glaucoma Drop (Dr. Irfan Kherani)
Q: What are the majority of reasons for not starting a patient on prostaglandin?
The majority of reasons include age, particularly for patients under 60, concerns about side effects such as hyperemia and changes in appearance, and patient preference against certain treatments.
36. Preservative Free Eye Drops (Dr. Miriam Kolko)
Q: What does the preservative-free movement signify for patient care?
The preservative-free movement is essential because it leads to safer options for patients, eliminating the toxic effects of preservatives, which can aggravate existing eye conditions.
36. Preservative Free Eye Drops (Dr. Miriam Kolko)
Q: What are some of the most common preservatives?
The most common preservative discussed is Benzalkonium chloride (BAC), which is highly toxic. Others, like polyquad, have less toxicity, but the concerns around preservatives remain significant.
