  • Cataract Coach is the Perfect Name

I have had the honor of being in ophthalmology for 44 years. I started with ICCE using a cryoprobe. We learned new techniques by going to meetings and by viewing grainy videos. Now thanks to doctors like Uday Devgan we have HD online videos and get to sit under the learning tree of some of the world’s top eye surgeons. I honestly wish I could give Dr. Devgan’s efforts more than 5 stars. Many patients worldwide see better because of him.

Thanks Dr. Devgan for your daily videos. I learned a lot. Now you have these amazing guests in this extended format. I didn’t know of Dr. Osher and Dr. Lindstrom. They gave really good advice for the important things in live. I really loved the episode with Dr. Oetting. He is such a humble and kind person. You hear how much he is dedicated to teaching.

Talking Points

Recent interactions between the hosts and their guests.

161: CataractCoach Podcast 161: Pedram Hamrah MD
Q: What is the big shift in thinking about the cornea's immune status?
The cornea is actively immune privileged, not just passively protected; it uses active mechanisms and a diverse set of immune cells to regulate immune responses and nerve interactions, which changes how we approach treatment.
160: CataractCoach Podcast 160: Jodi Luchs MD
Q: How do ophthalmologists get started with clinical trials, and what is the first step?
Start by building relationships with industry partners and startups, document your capability with a dedicated research coordinator, and demonstrate you can deliver quality data and patients. Attend conferences, engage with company reps, and consistently show up as a reliable site with the infrastructure to run trials.
159: CataractCoach Podcast 159: David Gunn MD
Q: What inspired you to pursue the CARES pathway and how did you first learn about it?
I learned about CARES by watching videos from early adopters like Soosan Jacob and then connected with mentors who guided my first cases, which helped me adopt a laser-assisted approach and accelerate my learning.
158: CataractCoach Podcast 158: Rebecca Metzinger MD
Q: What is your advice to young residents who struggle with cataract surgery?
Spend the day with an experienced mentor in the OR, focus on fundamentals, and emphasize safety and gradual skill-building rather than speed. Positive reinforcement and structured practice help build confidence and competence.
156: CataractCoach Podcast 156: John Jarstad MD
Q: Could you explain the John Jarstad zombie double tap technique for cataract surgery?
It involves overfilling the eye to create a controlled reservoir, then quickly entering and exiting the paracentesis with a small gauge cannula to enable faster fluid egress; this tends to keep the postoperative IOP in a safe, narrow range like 16–20, and it reduces day‑one spikes.
