An Update on Referred Muscle Pain with Dr. Fernando Exposto
Q: What future directions or projects are you most excited about in your work at Aarhus and with your team?
Continued exploration of CNS modulation in pain, refining experimental models, and moving toward personalized approaches that consider individual pain modulation profiles and genetic factors, with ongoing work in translating findings to clinical populations such as tension-type headache patients.
An Update on Referred Muscle Pain with Dr. Fernando Exposto
Q: How should clinicians calibrate palpation pressure in practice to avoid over-stimulating referred pain?
Clinicians can use standardized scales or devices like a palpator to calibrate pressure, but the key is to avoid overdoing the force and to consider whether the presence of referral changes the treatment plan; often, a clinician's judgment aligned with patient-reported familiar pain is sufficient.
An Update on Referred Muscle Pain with Dr. Fernando Exposto
Q: What questions motivated your Pain 2018 study comparing mechanical versus glutamate-induced referred sensations, and what were the main takeaways?
The study aimed to determine if the location of referred pain is shaped more by the stimulus force or by the modality. Results showed that the more painful the stimulus, regardless of modality, the higher the likelihood of referred pain, and the referred location tended to be consistent within individuals across sessions.
An Update on Referred Muscle Pain with Dr. Fernando Exposto
Q: What is the practical value of trigger-point injections for headaches or myalgia, and how do you view the mechanism behind their effect?
Injections can temporarily interrupt the pain cycle by activating endogenous pain systems, and needling may release neuromodulatory processes; however, the evidence for a lasting structural fix is limited, and benefits may wane after a short period.
An Update on Referred Muscle Pain with Dr. Fernando Exposto
Q: So why did you decide to compare NGF-induced myalgia with DOMS in the masticatory muscles, and what did you find?
We wanted to see which model better induces central nervous system changes and lasts longer. NGF tended to have longer-lasting effects and central changes, whereas DOMS did not show a clear central sensitivity increase, suggesting that translation to clinical TMD may be limited.
