Navigating the Transition of Obstetric Codes for Antepartum Care
Q: Would you mind talking a little bit more about these E&M codes and what they will be used for?
Antepartum care will be coded using standard E&M code sets based on location of service (office, outpatient, hospital), with specific ranges for new and established patients, and hospital/inpatient care; telemedicine has its own BR-based codes; overall this enables a more accurate reflection of care complexity and intensity.

Q: What should a clinician do if a payer won't recognize the modifier?
Be judicious in selection of cases, quantify the extra work clearly, and build relationships with payer provider relations staff and medical directors to advocate for appropriate reimbursement.

Q: How can providers document the extra work to support modifier 22?
Use time-based quantification or percentage increases over typical work, for example stating that the procedure took X minutes longer or was X percent more extensive than usual.

It should be considered only for cases where the work significantly exceeds the typical procedure, ideally in the top 10th percentile of the most challenging cases, with documentation quantifying the added work.

Q: Why isn't there a separate CPT code specifically for opportunistic salpingectomy at the time of cesarean delivery?
Because CPT 58611 was created as an add-on code for situations where sterilization occurs at cesarean or open abdominal procedures, and changing the code description or creating a new one would require a complex process including re-surveying valuations, which could risk reducing reimbursement.
