Q: What gives you hope about the future of value-based care and accountability in health care?
Hope comes from concrete evidence in research showing that full-risk arrangements with Medicare Advantage and other accountable models can deliver healthier outcomes and cost savings, along with the lived experiences of patients receiving higher-quality care under these models, which motivates continued investment and advocacy for broader adoption.
Healthcare Still Pays for Volume. Can It Really Change?
Q: What are the biggest incentives or obstacles preventing a shift toward category three or four accountability in the U.S. health care system?
The biggest obstacles are entrenched incentives to maximize volume, such as hospital bed utilization and procedures like surgeries, plus the reality that many stakeholders are currently doing well under the status quo, which reduces the urgency to change. The shift requires a fundamental reorientation of incentives and a commitment to build new systems around accountability.

Q: What book do you recommend to founders to learn to lead very well organization?
Radical Acceptance by Tara Brach, which emphasizes accepting all of who you are, including weaknesses and strengths, to develop compassion and better self-understanding.
