MS questions you’ve always wanted to ask a neurologist with Dr Aaron Boster | S8E18
Q: What is the relationship between DMTs and age, and how should patients weigh risks?
DMTs are helpful across ages and slow progression; risk-benefit assessments should contextualize cancer or infection risks with the substantial benefits of preventing MS activity and disability, with ongoing monitoring.
MS questions you’ve always wanted to ask a neurologist with Dr Aaron Boster | S8E18
Q: How should clinicians monitor disease activity beyond patient symptoms?
Clinicians should collect patient-reported outcomes, perform structural tests (MRI, OCT, bladder ultrasound), and use objective functional tests (MSFC, low-contrast visual acuity, SDMT, nine-hole peg test, timed 25-foot walk) to detect changes even when patients feel fine.
MS questions you’ve always wanted to ask a neurologist with Dr Aaron Boster | S8E18
Q: Please could you advise on menopause and taking HRT?
Estrogen levels fall during perimenopause and menopause, which can exacerbate MS symptoms; topical estrogen therapy is preferred because it reduces clot risk and cancer risk compared to systemic approaches, and it can help manage menopausal symptoms without worsening MS.
Beyond lesions: what advanced MRI may reveal about MS | S8E17
Q: What upcoming projects are you planning, and how might sodium MRI be integrated with other imaging techniques in the near future?
We plan to expand the sodium MRI work with larger, more representative volunteer groups, combine sodium measurements with MR spectroscopy to relate sodium changes to metabolite profiles, and explore faster MRI sequences that could maintain sensitivity while reducing scan times for diagnostics and monitoring.
Beyond lesions: what advanced MRI may reveal about MS | S8E17
Q: So to start off, could you give us a bit of an introduction of yourself and your work?
I'm Bhavana Solanky, an MRI physicist at the Queen Square MS Centre, focusing on developing MRI biomarkers to probe the brain at a cellular and chemical level, with the aim of measuring how MS affects brain chemistry and how this can inform treatment and monitoring.
