Talking Points

Recent interactions between the hosts and their guests.

Geriatric Physical Therapy: Building a Cash Based Practice | Dr. Sameep Contractor
Q: What one piece of advice would you give to a PT who is on the fence about starting a practice?
Start with baby steps and test the waters; don't expect immediate results. Focus on long-term value, keep a macro perspective on growth, and build visibility by getting in front of people and delivering real value before expecting referrals or patients to flood in.
Geriatric Physical Therapy: Building a Cash Based Practice | Dr. Sameep Contractor
Q: What are your thoughts on leadership versus management in running a practice, and how do you apply them?
Leadership is about sharing credit when things go well and taking collective ownership when things go wrong, fostering a “we” culture. Management involves organizing systems and people to align with shared goals, but both should be practiced with transparency and collaboration to grow the team together.
Geriatric Physical Therapy: Building a Cash Based Practice | Dr. Sameep Contractor
Q: What has the hiring and team-building experience been like, and how do you manage it day-to-day?
I stay involved in the initial visits to ensure quality, then transition roles to employees while maintaining weekly check-ins and clear communication. We use the same EMR, have shared policies, and I visit patients for re-evals as needed, ensuring continuity and quality of care.
Geriatric Physical Therapy: Building a Cash Based Practice | Dr. Sameep Contractor
Q: Sameep, could you start off by telling us a little bit more about you and where you started as a PT and what you do now?
I'm Dr. Sameep Contractor, a physical therapist who started Karma Rehab around 2019–2020, focusing on mobile, geriatric therapy. I began travel PT after graduating in 2002, moved back to Houston, and gradually shifted toward home health and independent living facilities before building Karma Rehab and hiring a small team to scale the practice.
Sales and Coaching in Physical Therapy: What Nobody Teaches You in PT School | Dr. Samantha Ritz
Q: Can you tell our listeners a little bit more about your experience with sales and how you felt when you first started and how you approach it now?
Sam explains that sales has a negative reputation but is an intrinsic, human skill; it involves listening, asking good questions, and structuring conversations so clients see value and can decide, leading to easier closings when you're a trusted guide.
