Annie, congratulations on launching your podcast — what an incredible start! You truly hit it right out of the gates.

I’ve been focusing on preventative brain health for the past five years, and I couldn’t agree more that food really is medicine. This opening episode does such a great job explaining the ten categories you believe are most important.

I often share information on brain health with friends, and now I feel like I finally have a wonderful resource to send along. Well done!

  • Talking back

I am pleased that I am already eating many of the foods from the many pyramids Annie suggests. My breakfasts typically consist of cottage cheese, a mixture of fresh berries, chopped pecans, a sprinkle of cinnamon, a tablespoon of flaxseed and a drizzle of local honey. Many people say to avoid dairy that I eat for protein. What is your opinion about cottage cheese? I do like that your comments are evidenced-based as that lends credibility to your words. I’ve followed you for years, have your cook... more

What You Can Do Now to Lower Your Risk of Alzheimer’s and Dementia with Dr. Kellyann Niotis
Q: What role do biomarkers play in prevention, and how should people think about testing for biomarkers like p-tau 217 or desmosterol?
Biomarkers are a key tool for understanding brain health trajectories over time. Tests like p-tau variants and desmosterol can provide insight into brain pathology and cholesterol synthesis, especially when done longitudinally to track trends rather than as a single snapshot. They guide personalized management, but are still evolving, and not all are appropriate for asymptomatic individuals yet.

Q: How do APOE4 and other genetic factors influence risk, and why is it important to understand that genes aren't destiny?
APOE4 does increase risk, but it is not deterministic. Most cases involve multiple factors—sex, family history, and modifiable risks. Knowing you carry certain genes can influence how you approach prevention, but it doesn't guarantee disease. This nuance helps people focus on controllable factors to potentially delay onset.
What You Can Do Now to Lower Your Risk of Alzheimer’s and Dementia with Dr. Kellyann Niotis
Q: What sparked your interest in preventive neurology and how did you come to focus on prevention rather than treatment alone?
I've always loved learning and was drawn to neurology because the brain controls everything from emotions to movement. Seeing relatives affected by aggressive dementia fueled my drive to find ways to prevent neurodegenerative diseases, not just slow them down. I realized early on that prevention requires a broad view across disorders and risk factors, not siloed specialties, and that led me to pursue a preventive neurology approach.
What Every Woman Should Ask Her Doctor About Brain Health with Barbie Boules, RDN
Q: When should someone consider testing for ApoE4 and what are the pros and cons?
Testing can motivate proactive health changes, but it can also cause anxiety if the individual is prone to worry. If knowing helps someone double down on protective behaviors, testing is valuable; otherwise it may be better to focus on actionable lifestyle changes regardless of status.
What Every Woman Should Ask Her Doctor About Brain Health with Barbie Boules, RDN
Q: Could you explain ApoE4 and how it affects risk for Alzheimer's in women?
ApoE4 increases risk, with one copy boosting risk about threefold and two copies elevating risk around twelvefold, but it is not deterministic. Carriers can still reduce risk through omega-3 intake, exercise, sleep, and metabolic health, especially in midlife.
