  • Great podcast, great doc!

So grateful for Dr Heather Hirsch and her podcast and social media presence educating us middle aged ladies. My favorite thing about her is that she is always positive and optimistic and takes the “doom and gloom” one of hormone change. Forever a listener of this podcast. ❤️

  • Awesome and well educated Dr!

I love listening to Dr. Heather Hirsch, she puts all of the information into understandable content, and I really appreciate her! I also took her prescriber course for practitioners like doctors, NPs, PAs and it was a fabulous course and I highly recommend that! Her content has made me feel comfortable prescribing HRT!

  • Lots of “Plugs”

In a field with very few experts, we need more people who want to genuinely help. I’m appreciative for the information, but I’m unsettled by how many plugs she’s always doing for her products, books, classes, etc… sure, she’s got to make a living, but people who really care don’t make you feel like their your only option for help.

  • Good

The topics are great! But I think the speaker jumps from topic to topic, a

jokes in between, that it makes it confusing to follow her or to get clear information.

Talking Points

Recent interactions between the hosts and their guests.

259: Ask Me Anything: Hormone Therapy Q&A
Q: Natasha asks how to get clinicians to prescribe testosterone and whether she is a good candidate.
Testosterone can be beneficial for low libido, mood, brain fog, and joint pain in postmenopausal women when used transdermally at a tenth of a male dose; eligibility depends on overall hormone balance and absence of contraindications; clinicians should consult consensus guidelines and consider off-label use when appropriate.
259: Ask Me Anything: Hormone Therapy Q&A
Q: Rachel asks why her heavy bleeding continues on estradiol patch with progesterone and whether cycling can help.
Cycling progesterone can reduce continuous bleeding and create a predictable withdrawal bleed; if bleeding persists, consider decreasing estrogen, increasing progesterone, or trying an intrauterine device delivering progesterone; bleeding is common in perimenopause during HRT.
259: Ask Me Anything: Hormone Therapy Q&A
Q: Jen asks why she's absorbing less on a higher patch dose and whether switching to gel is equivalent.
Absorption and formulation differences mean patch and gel are not always equivalent; if nighttime symptoms worsen when switching, consider returning to the previous regimen and adjusting to a specific gel dose (e.g., 0.75 or 1.0 mg) or using gel at night for peak effect.
259: Ask Me Anything: Hormone Therapy Q&A
Q: Patricia Wright asks about adjusting her estrogen patch dose and progesterone for sleep in the context of osteopenia and sleep disruption.
The clinician explains there is no strict age limit for patch dose, that higher doses do not inherently increase risks like clots or cancer, and suggests focusing on symptom control and overall well-being; increasing estrogen may help sleep, while progesterone adjustments depend on tolerability and absorption.
235. Project Happiness, with Author and philosopher Stephanie Harrison
Q: What do we know about happiness and finding?
Starting to name and notice how old happy culture affects us can facilitate the transition to new happy alternatives, grounding responses in connection and community.
