  • Use of “in her prime”

Not loving that only women in their 20s and 30s are considered “in her prime” in the latest episode. I thought we were trying to dismantle the narrative that women have moved beyond their prime after age 40.

  • Love this doctor so much!!

Dr Hirsch is amazing and so informative. I truly wish I could afford to join her program, The Collaborative because I believe in her approach to midlife medical treatment for women. For now I am stuck with Midi.

  • Love her content!

I follow Dr Hirsch on Instagram & Apple Podcasts. I just love everything she stands for- supporting women and teaching other clinicians how to support women. Her podcasts are very educational but not in a boring way. I appreciate her energy and the way she speaks to her audience- very engaging and lively.

  • Fear Monger

She ramps up fear around certain modalities and then genuinely wonders - in the next breath- why women are afraid of HRT. I’m so tired of this.

  • Great info.

Dr. Hirsch asks the guest, “why aren’t more people getting these treatments?” and I wish one of them had said that it’s because they are generally not covered by insurance. The vast majority of people cannot afford to pay for their medical care out-of-pocket. This knowledge and these treatments need to become part of mainstream medical care that is either covered by insurance or somehow more attainable. Women desperately need this information and this care!!!

Talking Points

Recent interactions between the hosts and their guests.

268. A Midlife Health Masterclass with Dr. Heather Hirsch, Dr. Alexandra Sowa & Rachel Goldman, Ph.D.
Q: What is the cognitive-behavioral approach to breaking the nothing works spiral?
Identify automatic negative thoughts, pause to assess helpfulness, reframe toward actionable steps, and pair with labs, education, and support to move from frustration to proactive care.
261. Why Midlife Women Have More Joint Pain and What Actually Helps
Q: What exactly is regenerative medicine offering for these conditions, and when do you choose PRP vs bone marrow concentrate?
PRP is usually first-line for tendon/ligament issues; bone marrow concentrate is reserved for larger cartilage or complex tears where PRP hasn't yielded adequate results. Both are guided by ultrasound and paired with PT for best outcomes.
261. Why Midlife Women Have More Joint Pain and What Actually Helps
Q: How do patients typically reach your practice, and what happens when PT and conservative care don't help?
Patients often start with more common care pathways, but when pain and dysfunction persist, we diagnose precisely (e.g., which rotator cuff muscle is involved) and coordinate imaging, injections, and PT in a team setting to avoid unnecessary surgery.
259: Ask Me Anything: Hormone Therapy Q&A
Q: Natasha asks how to get clinicians to prescribe testosterone and whether she is a good candidate.
Testosterone can be beneficial for low libido, mood, brain fog, and joint pain in postmenopausal women when used transdermally at a tenth of a male dose; eligibility depends on overall hormone balance and absence of contraindications; clinicians should consult consensus guidelines and consider off-label use when appropriate.
259: Ask Me Anything: Hormone Therapy Q&A
Q: Rachel asks why her heavy bleeding continues on estradiol patch with progesterone and whether cycling can help.
Cycling progesterone can reduce continuous bleeding and create a predictable withdrawal bleed; if bleeding persists, consider decreasing estrogen, increasing progesterone, or trying an intrauterine device delivering progesterone; bleeding is common in perimenopause during HRT.
