  • Slow down and balance your mics

The content is great. Such valuable info. But it’s so difficult to understand since one docs voice is so much louder and her mic isn’t balanced with the rest. And the other two don’t get a chance to finish their thoughts because she interrupts and talks over everyone.

  • Good but…

You guys have lots of knowledge and I love learning and I want to listen, but you trample each other with your words and it makes it hard to follow and understand . Please respect each other’s turn, speak slower and give each other the space to finish before going on to the next thing. I want to listen and learn but it becomes a squabble of words and I want turn it off.

Talking Points

Recent interactions between the hosts and their guests.

#79 -Unlocking the Secrets of Fiber and Prebiotics with Dr. Mark Davis
Q: What is the current view on microbiome testing to guide prebiotic therapy?
Many clinicians find microbiome data not reliably actionable yet, with a large portion of results lacking clear guidance; decisions are often based on clinical symptoms and patient response rather than test results.
#79 -Unlocking the Secrets of Fiber and Prebiotics with Dr. Mark Davis
Q: How do you deliver prebiotics to patients who have trouble tolerating higher fiber doses?
Start with tolerable, slowly ramped doses using partially hydrolyzed guar gum (sun fiber) or psyllium, then gradually increase while monitoring for bloating, with options to use powders in foods like yogurt or smoothies to improve compliance.
#79 -Unlocking the Secrets of Fiber and Prebiotics with Dr. Mark Davis
Q: Why explore blends instead of single prebiotics?
Blends are favored by experienced clinicians because they can target multiple microbial niches and offer a more tailored approach to microbiome modulation, with Sharon Eldridge's work serving as inspiration for developing combinations that could address patient-specific dysbiosis.
#78 -Fecal Microbiota Transplant with Dr. Mark Davis
Q: What are typical remission or response rates for C. diff with FMT, and what about other conditions?
For C. diff, remission rates are typically in the high 80s to 90s depending on the study and protocol; for other conditions like UC or Crohn's disease, responses are variable, with some patients experiencing significant benefit and others not, underscoring the need for individualized risk-benefit assessment.
#78 -Fecal Microbiota Transplant with Dr. Mark Davis
Q: What are the main administration routes and how do you decide which to use?
Routes include colonoscopy, enemas, and oral capsules; while colonoscopy and enemas may have slightly higher efficacy in some contexts, capsules offer convenience and good efficacy, so the choice depends on patient tolerance, accessibility, and safety considerations.
