    I am finding this podcast incredibly informative as someone who is just starting to realise that so many of my health problems fit with MCAS. At the same time, I’m on a journey of self discovery as a late diagnosed autistic& ADHD woman. I’m sure there’s a link here- sensitive mind/sensitive body, and you allude to this a few times yourselves.     Loving listening to this podcast in fact it’s a favourite at the moment. I’m in the uk and have EDS with pots and mast cell problems with allergies and digestive issues. Joint pain and diagnosed with fibromyalgia and this is helping me as I was feeling very alone and misunderstood. Love listening to all the medical drs and getting myself fully informed on this condition. After getting covid 2022 I ended up with long covid and slowly trying to get back to a functioning place. Q: What do you do first when a patient is highly reactive—stabilize mast cells or treat the infection first?
It's highly individualized; often you start with stabilizing the mast cells because symptoms are driven by MCAS, but you may need to treat the infection if it remains a persistent trigger. Some patients respond to mast cell–targeted therapies alone, while others require a combination approach, sometimes starting with immune support or IV vitamins and gradually escalating to more targeted antimicrobial or adjunctive therapies as tolerated.

Q: Do you have any tests that you would say are either good choices or bad choices since there's so many different tests out there?
Antibody testing is not reliable in MCAS patients; it can give false positives or miss active infection due to immune dysfunction. Molecular tests like PCR or FISH are more reliable for detecting active infection, though they can be expensive; repeating tests may be necessary because the pathogens may not be present in the blood at all times.

Q: Can endometriosis and POTS be diagnosed as two mast cell–related system involvements, and how should patients pursue testing?
Yes, two or more systems being involved supports MCAS diagnosis; testing for mediators can help but clinical diagnosis remains important, especially when testing access is limited; seek care from a mast cell–savvy clinician.

Q: What recommendations do you have for someone diagnosed with endometriosis and possibly MCAS, especially regarding surgery versus medical management?
Stabilize and assess inflammation and MCAS first; optimize before elective surgery to improve recovery, while acknowledging urgent cases may require proceeding with surgical plans; consider GLP-1 implications and broader anti-inflammatory approaches when appropriate.

Q: What practical steps can patients take at home to support membrane health?
