Love the mission. Nice review with helpful explanations of answer options. The ad lib style of the hematology 1-3 episode is very distracting and hard to follow the case, question, and answer choices, however. I prefer the straightforward questions to mimic test taking

Talking Points

Recent interactions between the hosts and their guests.

Ophthalmology 10-12
Q: Which of the following is the most appropriate next step in the management of this patient with a chemical eye injury?
Immediate topical anesthetic with proparacaine followed by copious irrigation and stabilization of the ocular pH before any further examination.
Orthopedics 7-9
Q: Question 3, Orthopedics 9. A 24-year-old man snowboarder sustains an anterior shoulder dislocation, typical findings include abducted externally rotated arm and loss of deltoid contour; axillary nerve injury is the most common complication.
The correct answer is A: Axillary nerve damage, which can cause decreased sensation over the lateral deltoid and weakened deltoid function; this injury accounts for the majority of anterior shoulder dislocations.
Orthopedics 7-9
Q: Question 2, Orthopedics 8. A 16-year-old gymnast presents with acute knee injury after a non-contact twisting mechanism with lateral patellar dislocation; immediate management involves closed reduction with medial patellar pressure and knee extension.
The correct answer is B: Closed reduction via medial pressure on the patella with knee extension, followed by immobilization and physical therapy; this acutely reduces the dislocation and prevents further chondral damage.
Orthopedics 7-9
Q: Question 1, Orthopedics 7. A 19-year-old man is admitted to the hospital for surgical management of a closed comminuted femoral fracture sustained in a motor vehicle accident 24 hours ago.
The correct answer is C: Intraretinal hemorrhages and intravascular fat globules, consistent with fat embolism syndrome presenting with hypoxia and neurologic symptoms; fat emboli can cause characteristic ocular findings and the condition typically manifests 12-72 hours after long bone fractures.
Neurology 13-15
Q: Question 3 asks for management of an acute ischemic stroke when thrombolysis is not indicated due to time window.
Aspirin with clinical monitoring is the most appropriate initial management; permissive hypertension is used when thrombolysis is not pursued.
