Love the mission. Nice review with helpful explanations of answer options. The ad lib style of the hematology 1-3 episode is very distracting and hard to follow the case, question, and answer choices, however. I prefer the straightforward questions to mimic test taking

Talking Points

Recent interactions between the hosts and their guests.

ENT 7-9
Q: Question 3, ENT 9: A 32-year-old with chronic ear discharge, a pearly white mass in the middle ear, and conductive hearing loss; what is the next best step?
The correct choice is surgical excision of the lesion (cholesteatoma) because it is destructive and requires removal to prevent complications, rather than relying on antibiotics or observation.
ENT 7-9
Q: Question 2, ENT 8: A 27-year-old with swimming-associated ear pain and tragus tenderness; what is the initial treatment?
The correct choice is topical antibiotic ear drops, as acute otitis externa is treated locally with topical therapy rather than systemic antibiotics unless the infection is severe or invasive.
ENT 7-9
Q: Question 1, ENT 7: A six-year-old with acute mastoiditis presents with fever, postauricular swelling, and a bulging tympanic membrane; which is the most appropriate next step in management?
The correct choice is high-dose intravenous antibiotics with otolaryngology consultation, because mastoiditis requires hospital-level IV therapy and specialist involvement rather than outpatient management or limited imaging.
Hem-Onc 4-6
Q: Question 3: A patient develops fever a few hours after transfusion with normal hemolytic parameters; what explains the fever?
Release of cytokines from the transfused blood leads to fever without hemolysis, constituting an acute febrile non-hemolytic transfusion reaction.
Hem-Onc 4-6
Q: Question 2: In hereditary hemorrhagic telangiectasia (Osler-Weber-Rendu), which feature is most characteristic and typically present with mucocutaneous telangiectasias and epistaxis?
Pulmonary arteriovenous malformations are a key associated vascular abnormality in HHT and contribute to the clinical spectrum including epistaxis and GI bleeding.
