  • Dangerous advice

I have only listened to a few…. Thank heavens! Their episode 381 and recommendation on how much vitamin D one should take (600-800 iu) is grossly inaccurate. Better to listen to someone who has better more accurate information.

  • Very informative and easy to follow

This podcast does a great job of sharing the science and breaking down concepts relevant to all humans but as a movement professional I find the topics and information especially salient. I often share with coworkers and I’m so happy you put this out. Thanks for all you do to debunk myths and level up knowledge.

  • The best fitness podcast out there

Drs. Feigenbaum and Baraki put out the most science-backed, informative podcast out there. They only present the most important information in a very digestible way and they’re both so articulate. As a doctor, I know just enough about health and fitness to know when I’m hearing pure gold. They’re a refreshing change from other podcasts that platform quacks. And they’re funny too. I’ve stopped listening to other podcasts because I can trust what these doctors say.

  • Real Information Based on Actual Evidence

The Barbell Medicine podcast (and their website, and programs, and supplements) are actually based on a nuanced understanding of the actual literature. What a novel concept! It’s a breath of fresh air in a space where the loudest voices tend to be the dumbest.

Talking Points

Recent interactions between the hosts and their guests.

Episode #389: Your Liver Enzymes Are Elevated — But It Might Not Be Your Liver
Q: What is your differential diagnosis in this case?
Initial differential emphasizes ongoing exposure/toxin, fatty liver disease, infection, and autoimmune etiologies; given five months of progressive enzymes, emphasis shifts toward a non-acute process with possible long-term exposures, fatty liver despite lean habitus, or less likely autoimmune disease; ongoing evaluation should include repeating tests and considering CK/LDH to distinguish muscle from liver sources.
Episode #389: Your Liver Enzymes Are Elevated — But It Might Not Be Your Liver
Q: Can you take us through the different labs contained in a liver panel and explain them to the audience?
Liver panels separate hepatocellular markers (ALT/AST) from cholestatic markers (ALK phosphatase, bilirubin) and add synthetic function tests (albumin, INR) to gauge overall liver performance; ALT is more liver-specific but not exclusive, AST also comes from muscle; ALP and bilirubin relate to bile flow; GGT can help differentiate hepatobiliary vs bone sources but is not definitive.
Episode #389: Your Liver Enzymes Are Elevated — But It Might Not Be Your Liver
Q: First, what do you think about this patient?
The patient has a persistent, mild pattern of liver chemistries with a mixed hepatocellular and cholestatic signal, and the clinician should consider ongoing exposure, fatty liver disease, or less likely autoimmune disease, while keeping in mind a possible exercise-related elevation; rational follow-up includes targeted serologies and imaging before pursuing invasive tests.
Ozempic & Alcohol, The Trap Bar Myth, and A Medical Mystery | Barbell Medicine AMA Teaser
Q: Am I missing critical gains by using the trap bar to save my lower back?
You're not missing any critical gains; it's about personal preference and overall training goals.
Ozempic & Alcohol, The Trap Bar Myth, and A Medical Mystery | Barbell Medicine AMA Teaser
Q: How do you view heavy alcohol use alongside GLP-1 medications?
The aggregate health risk is lower even if the alcohol variable remains constant, and GLP-1 medications can actually improve metabolic health, liver function, and cardiovascular risk profile.
