Q: How might what we know about EBV and MS be used to treat or even prevent MS?
Strategies include targeting EBV-infected B cells with therapies that deplete B cells, vaccine approaches to prevent EBV infection or modulate the immune response, antiviral therapies against EBV life cycles, and broader approaches to reshape the immune response to EBV to prevent autoimmune activation in the CNS.

Q: And you mentioned this a bit, but what have we learned in recent years about the mechanisms that link EBV to MS?
There are four main mechanisms proposed: molecular mimicry between EBV antigens and CNS autoantigens; EBV-driven changes in B cells that migrate to the CNS and promote inflammation; immortalization of autoreactive B cells leading to autoantibody production; and EBV-infected B cells acting as potent antigen-presenting cells that may trigger autoreactive T cell responses. These mechanisms may operate differently across MS patients, and it's still unclear which is most dominant.

Q: What are your immediate research priorities coming out of this study?
The priorities include expanding collaborations to collect more patients across CSF-leak subtypes, refining animal models to dissect abnormal cellular behaviors, and pursuing treatment strategies that modify cellular responses rather than solely addressing structural tissue integrity.

Q: Do you think fibrillin-2 genetic testing has a role in clinical practice today?
It is early in the process; routine testing isn't standard yet, but testing could be considered for families at risk or for understanding broader genetic risk, with the caveat of incomplete penetrance and the need for further validation across larger cohorts.

Q: Can you summarize the key genetic finding and what makes it particularly compelling?
The study identifies deleterious variants in fibrillin-2 associated with CSF leaks in a substantial subset of patients, representing their first genetic link to spontaneous leaks independent of overt connective tissue disorders, and suggesting a mechanistic role tied to integrin interactions and extracellular matrix signaling.
