Q: What is the current state of biomarkers and how do they help in risk assessment for carriers?
Biomarkers specific to GBA1-related PD are still lacking; enzyme activity in blood shows group differences but is not definitive for individuals. CSF markers and imaging biomarkers are not unique to GBA PD, so the focus remains on genetic variant type to guide prognosis, with seed amplification assays for alpha-synuclein representing a promising area for risk stratification and longitudinal monitoring.

Q: Could you please explain the clinical profile of patients with GBA1 variants and how they differ from idiopathic PD?
Patients with GBA1 variants generally show an earlier onset and a tendency toward faster progression with more pronounced non-motor symptoms, especially cognitive dysfunction, and this is driven largely by the severity of the GBA1 variant. However, there isn't a single clinical feature that identifies a GBA1 carrier, so genetic testing and careful family history are important for risk assessment and counseling.

Q: So, how do you distinguish between cognitive resilience and cognitive reserve? And what do you mean by brain maintenance and brain reserve?
Cognitive reserve is the total cognitive resources a person has, including intelligence and education, and it serves as the basis for resilience; brain maintenance refers to the absence of brain changes over time, while cognitive resilience is the ability to perform cognitively well despite pathology. Both concepts can be used in tandem within a study.

Q: Could you explain briefly the pathological changes that occur in Alzheimer's disease and the challenges to detecting them in vivo?
Alzheimer's begins with amyloid plaque buildup followed later by tau pathology spreading through the brain; detecting these changes in living people is hard because the markers are distributed unevenly, vary between individuals, and many other diseases can cause cognitive symptoms, complicating attribution to Alzheimer's. Blood-based biomarkers offer less invasive windows into these processes and can help in early identification and stratification when combined with imaging and clinical assessment.

Q: Based on current evidence and guidelines, who should receive preventive treatment for migraine and when?
Any person with migraine fulfilling certain conditions, such as having four or more monthly headache days, is a candidate for preventive treatment.
