Talking Points

Recent interactions between the hosts and their guests.

June 2026 Recall: Topics on Parkinsonian Disorders
Q: How should clinicians communicate a PD diagnosis given potential changes later?
Clinicians should present Parkinson's disease as the best working diagnosis today, be honest about the possibility of future changes, and emphasize ongoing follow-up and reassessment.
Inflammation, Limbic White Matter, and Symptoms After Repetitive Head Impacts in Retired Football Players
Q: Could you elaborate on the different biomarkers and the radiographic imaging or diffusion tensor imaging that were used in the study?
The study used fluid biomarkers from plasma and CSF, focusing on inflammation-related proteins like IL-6, TNF-alpha, and GFAP, complemented by diffusion tensor imaging to assess limbic white matter microstructure. The aim was to link inflammation with white matter changes and then to cognition, acknowledging DTI provides a snapshot of brain structure rather than a disease-specific signature; biomarkers were informative but not yet ready for clinical decision-making.
Inflammation, Limbic White Matter, and Symptoms After Repetitive Head Impacts in Retired Football Players
Q: Could you please give a brief rundown on the distinction between Traumatic Encephalopathy Syndrome and Chronic Traumatic Encephalopathy, or CTE?
TES is a research framework anchored to clinical features and prior head trauma exposure, used to predict who might have CTE, which is a neurodegenerative disease defined by brain pathology. Symptoms and brain abnormalities can diverge, so TES helps describe clinical features while CTE reflects underlying neurodegeneration; they are related but not the same, and a patient can meet TES criteria without having CTE, or have CTE without meeting TES criteria.
Clinical Heterogeneity and Candidate Biomarkers in POLG-Related Mitochondrial Disease
Q: Cristina, let's start really basic. When we say mitochondrial disease, what do you want clinicians to actually picture?
Mitochondrial disease is a highly heterogeneous group of genetic disorders where the oxidative phosphorylation system is impaired, and because it involves both nuclear and mitochondrial genomes, it can affect almost any organ, tissue, or age group, making diagnosis and management challenging.
2026 AAN President's Award Recipient - Dr. Walter J. Koroshetz
Q: What is the Brain Initiative and why is it transformative?
The Brain Initiative targets brain circuits, developing tools to map, monitor, and modulate them; it has produced powerful technologies that enable circuit-level understanding and treatment approaches, akin to a neuroscience-scale revolution.
