What a brilliant podcast. As a practicing Consultant Neurologist in the UK I am a regular listener.

The three podcast formats are all brilliant.

Editors, Editors pick and case discussions each have engaging and passionate presenters. The trainees on the case based discussions are great. I always encourage my colleagues and trainees to listen to this pod!

  • Thank-you Geraint and Phil

The editors do a great job, much in the manner of ‘The Rest Is History’ providing an informative but also entertaining and often amusing exchange. In the manner of an aperitif, with the prospect of an excellent meal ahead, the highlights whet our appetite for the journal’s upcoming issue, with all the convenience of audio for continuing medical education.

Talking Points

Recent interactions between the hosts and their guests.

Radiologically isolated syndrome: managing the preclinical phase of MS
Q: Could you discuss the role of biomarkers and imaging advances in monitoring RIS going forward?
Biomarkers such as oligoclonal bands in CSF, neurofilament light chain in serum, and imaging advances like susceptibility-weighted imaging and possibly OCT, could help monitor progression or conversion risk and guide decisions about surveillance frequency and treatment.
Radiologically isolated syndrome: managing the preclinical phase of MS
Q: What are the key radiological features that help you pull RIS apart from other white-matter changes like migraine or small vessel disease?
Demyelinating lesions should be typical and in the right locations (juxtacortical, periventricular, infratentorial), have appropriate size and shape (ovoid, possible Dawson's fingers), and may include spinal cord involvement or gadolinium-enhancing lesions; central vein signs and the SeLX rule can aid in identifying MS or RIS with high sensitivity and specificity.
Radiologically isolated syndrome: managing the preclinical phase of MS
Q: How has RIS evolved with the McDonald Criteria, and what does this mean for diagnosing MS in RIS patients?
The McDonald Criteria have shifted toward a biological diagnosis, recognizing that some RIS patients have higher risk factors (multiple lesions in topographies, positive oligoclonal bands, essential vein signs) that justify considering MS and potentially starting disease-modifying therapy rather than waiting for a clinical syndrome.
Radiologically isolated syndrome: managing the preclinical phase of MS
Q: Audrey, could you introduce yourself to our listeners and explain what radiologically isolated syndrome is?
RIS is the incidental discovery of MRI changes consistent with demyelination in someone with no signs or symptoms of MS, often found when imaging for another reason such as migraine; the challenge is distinguishing RIS from early MS and deciding how to monitor or treat.
A face in the hedge: Dementia with Lewy bodies
Q: What kinds of interdisciplinary support should be included in the management of DLB?
Essential support includes physiotherapy, social work, occupational therapy, speech therapy, and nutritional counseling due to the various needs of DLB patients.
