Talking Points

Recent interactions between the hosts and their guests.

Refractory Headache Disorders, New Consensus, and Emergency Department Migraine Guidelines - Part 1
Q: Why don't you tell us a little bit about the consensus definition?
Jennifer outlines the structure of the consensus definition, including criteria A, B, and C and how they integrate to classify refractory migraine, with emphasis on ensuring accurate differentiation from other diagnoses.
Refractory Headache Disorders, New Consensus, and Emergency Department Migraine Guidelines - Part 1
Q: You've done a lot of work in the area of refractory migraine. Why don't you tell us why you felt there need to be clarification on the definition?
Jennifer explains the motivation for standardizing the definition, noting the lack of clear patient identification, insufficient treatment guidance, and the need to propel research forward.
Seizures and Epilepsy in Patients With Untreated Cerebral Cavernous Malformations
Q: Abel, can you give our listeners a rundown of the most exciting findings and how it can change practice?
The study shows that CCM patients with a first unprovoked seizure should be diagnosed with epilepsy and started on anti-seizure medication, because many achieve long-term seizure freedom with medical therapy, underscoring a shift toward early treatment after a first seizure.
Refractory Headache Disorders, New Consensus, and Emergency Department Migraine Guidelines - Part 2
Q: Can you give the summary of those findings?
We reviewed 26 new randomized trials in the ED, graded them, identified two Grade A medications to offer (Prochlorperazine IV and Greater Occipital Nerve Blocks) and one Grade A to avoid (IV hydromorphone); several Grade B options were identified (Dexketopropan, Ketorolac, Metoclopramide, Sumatriptin Subcutaneous, Supraorbital Nerve Blocks), with the goal of standardizing emergency department migraine care.
Diagnostic Yield of Reanalysis After Nondiagnostic Genome Sequencing in Infants With Unexplained Epilepsy
Q: Can you tell us what you found briefly and why it's important for neurology care?
We found that implementing reanalysis for infants or any individual with epilepsy within a year or two of non-diagnostic testing may be useful, as it led to identifying genetic diagnoses in about 5% of cases.
