  • Bring back the teaching!

Huge fan of continuum. I still think it’s the best print for high-yield review of neurology. Often very thought out and helpful to every day clinical practice. Unfortunately the evolution of continuum podcast has become less teaching. Not all of the episodes but more than listeners care for. Bring back the life long learning, we are here to soak it up!

  • Not as informative

The previous format was much more informative, it had nice bits of advice and pearls regarding differential diagnosis and treatment… it resembled more and complimented the written article.

The new format sounds more like an informal and social interaction, not so much clinically relevant information.

  • No longer useful

Continuum Audio no longer has useful clinical information. I used to learn things from continuum audio but now the interviews focus on the interviewee and don’t contain any clinical content

  • Enjoy the podcasts!

The podcasts make it much easier to access Continuum Audio, but sometimes the podcasts come across with more of an infomercial quality than the medical content they used to have. I think it’s important to advertise the journal, but to me, as a long time subscriber, the new podcasts come across as more of an advertisement.

  • Easier to access but limited helpful information

While much easier to access compare to Audio digest app, the podcast has only limited helpful information about the topics discussed. This might be a useful add on to the audio digest app, but definitely shouldn’t be a replacement imo

Talking Points

Recent interactions between the hosts and their guests.

Neuromyelitis Optica Spectrum Disorder With Dr. Sara Mariotto
Q: What is the prognosis and how has it changed with modern therapies?
NMOSD is typically relapsing, but prognosis has improved significantly due to early recognition and use of targeted immunosuppressive therapies, leading to better outcomes for many patients.
Neuromyelitis Optica Spectrum Disorder With Dr. Sara Mariotto
Q: What are the red flags that might suggest an alternative diagnosis to NMOSD, especially in seronegative patients?
A very hyperacute onset and a progressive course independent of relapses are red flags that should prompt consideration of other etiologies rather than classic NMOSD.
Neuromyelitis Optica Spectrum Disorder With Dr. Sara Mariotto
Q: What are the boundaries between MOGAD and NMOSD, and when should NMOSD be considered in testing for antibodies?
Antibody testing for aquaporin-4 should be targeted to cases with atypical optic neuritis, myelitis, or brainstem syndromes, rather than all typical MS presentations, because the test is very specific but not necessary for every case. Testing should be guided by clinical suspicion and atypical features.
Diagnostic Neuroimaging Biomarkers for Multiple Sclerosis With Dr. Jiwon Oh
Q: What does the paramagnetic rim lesion tell us about MS biology, and how specific is it to MS compared with other white matter diseases?
Pearls (paramagnetic rim lesions) reflect chronic active lesions driven by ongoing microglial activity, and while highly specific for MS (around 90-95% specificity), they are less sensitive than the central vein sign and are not present in all MS cases.
Diagnostic Neuroimaging Biomarkers for Multiple Sclerosis With Dr. Jiwon Oh
Q: Does that mean the neuroradiologist needs to look at every single lesion and then count how many have the central vein sign? Or is there an easier way to do this?
There is an easier criterion called Select 6: if you have more than 10 lesions, show that six have a central vein; if 10 or fewer lesions, show that more than 50% have a central vein. This simplifies the assessment while preserving diagnostic performance.
