  • Useful info even just for pet owners looking for further info :)

I have a dog who’s had allergies since she was younger. Only 4 and has been dealing with a paw infection since November 20th. She got it cultured last week and just found out it’s MRSP. Episode 63 was really useful, but have MRSP cases in Oregon risen more over the years?

326. New JAK Inhibitor for Allergic Dogs? What Vets Need to Know About Numelvi
Q: How quickly does Numelvi work and what should clinicians monitor?
Numelvi can start reducing itch within 2-4 hours with meaningful relief by day one and further improvement by week one; clinicians should monitor for infection and perform cytology to ensure full efficacy, adjusting therapy if infection is present or if response wanes.
326. New JAK Inhibitor for Allergic Dogs? What Vets Need to Know About Numelvi
Q: Who are the ideal candidates for Numelvi?
Ideal candidates include younger dogs requiring more than simple antipruritic control, dogs on multiple therapies, those not fully controlled with other treatments, and dogs with allergic dermatitis of various causes needing a first-line JAK option.
326. New JAK Inhibitor for Allergic Dogs? What Vets Need to Know About Numelvi
Q: What is Numelvi and how does it work differently from other JAK inhibitors?
Numelvi is a JAK-1 selective inhibitor (10-vicitinib) designed to target the primary drivers of itch and inflammation in allergic dermatitis while minimizing off-target effects; it is also approved for six-month-old dogs, offering a broader starting point.
324. Biopsy Timing, Infectious Differentials & Managing Sterile Nodular Panniculitis
Q: Hi Dr. Bourgeois, I really enjoy your podcast and do cytology on most of my skin cases now. For small-yield samples, like claw folds or flaky samples, how do you make sure the sample stays on the slide during staining?
The host walks through sampling techniques, highlighting toothpick scrapings for claw folds, ensuring a thin, pressed-on layer, and switching methods (tape, direct smear) when yield is poor; emphasizes not overloading slides with debris and allowing slides to dry or fix as needed, with practical steps to optimize slide adherence and readability.
283. GRAB BAG: Otitis
Q: How do you maintain allergic ears that flare but don't necessarily have infection?
Use topical steroids to control inflammation, maintain with regular seruminolytic flushes, adjust flushing frequency based on symptoms, and educate owners on home care to prevent progression to infection.
