Talking Points

Recent interactions between the hosts and their guests.

E13. Unpacking RANZCOG’s Clinical Guideline for Prenatal Screening for Fetal Anomalies (audio)
Q: What makes pre- and post-test counselling important in this guideline?
Counselling helps patients understand what the tests assess, the possible results (including uncertain or mosaic findings), and the implications for decision-making, ensuring informed choices before and after testing.
E13. Unpacking RANZCOG’s Clinical Guideline for Prenatal Screening for Fetal Anomalies (audio)
Q: When should diagnostic testing like CVS or amniocentesis be considered versus continuing with screening tests?
Diagnostic testing is recommended for more certainty, but the choice depends on timing, patient preferences, clinician experience, and the specific clinical context; some patients may opt for screening first, and in high-risk cases, diagnostic testing may be pursued sooner.
E13. Unpacking RANZCOG’s Clinical Guideline for Prenatal Screening for Fetal Anomalies (audio)
Q: Why is the guideline emphasizing universal access to prenatal screening for all pregnant women?
Because offering screening to all women helps identify conditions early, provides clinical and psychological benefits, and prevents assumptions about a patient's attitudes toward abortion from guiding care.
E14. Unpacking the ADIPS 2025 Consensus Recommendations for the Screening, Diagnosis, and Classification of Gestational Diabetes (audio)
Q: To start us off, Alexis, could you please run us through the key changes in the new Adips guidelines?
Alexis explains that early pregnancy screening is clearer and often uses HbA1c to identify people at risk, with diagnostic thresholds raised so fewer women are diagnosed with gestational diabetes. All pregnant women without a prior diabetes diagnosis should still be screened at 24–28 weeks with an OGTT, and the use of HbA1c is mainly for type 2 diabetes in early pregnancy rather than gestational diabetes.
E14. Unpacking the ADIPS 2025 Consensus Recommendations for the Screening, Diagnosis, and Classification of Gestational Diabetes (video)
Q: To start us off, Alexis, could you please run us through the key changes in the new Adips guidelines?
Alexis explains that early screening now uses HbA1c to reduce the need for multiple GTTs, thresholds for diagnosing GDM have increased, OGTT remains the gold standard, and there is a shift to focus care on those most likely to benefit, with a continued commitment to screening all pregnant women at 24-28 weeks with OGTT.
