Q: Do you see a role for GEP in guiding imaging surveillance or decisions about adjuvant therapy in melanoma?
The current data do not demonstrate that GEP improves surveillance strategies or helps decide on adjuvant therapy in a prospective manner. There is interest in the possibility that GEP could help refine risk within subgroups and potentially predict response to therapies, but evidence is not sufficient yet to alter standard management.

Q: Based on the evidence you reviewed, does GEP provide added prognostic value beyond traditional factors like Breslow depth and ulceration?
GEP technologies did not consistently show added value beyond traditional histopathologic factors across all stages, particularly in very thin lesions. In some non-thin or specific subgroups, there may be potential advantages, but overall, the data do not support routine reliance on GEP to replace or supersede established clinical-pathologic features.
