Talking Points

Recent interactions between the hosts and their guests.

Pink Toes and Blue Brain on VA ECMO (North South Syndrome on ECMO)
Q: What is differential hypoxia (North-South syndrome) and why does it occur on VA ECMO?
Differential hypoxia occurs when desaturated blood ejected by a still-functioning left ventricle mixes with oxygenated ECMO return flow, creating a watershed in the aorta that deprives the upper body and brain of oxygen while the lower body is well perfused by ECMO; it is driven by inadequate mixing and positioning of cannulas.
Pink Toes and Blue Brain on VA ECMO (North South Syndrome on ECMO)
Q: How is sepsis-induced myocardial dysfunction diagnosed?
Diagnosis relies on bedside echocardiography showing reduced LV ejection fraction and chamber dilation, supported by biomarkers such as elevated troponin and natriuretic peptides, plus trending lactate and other cardiac/biomarker tests.
Pink Toes and Blue Brain on VA ECMO (North South Syndrome on ECMO)
Q: What is sepsis-induced myocardial dysfunction?
Sepsis-induced myocardial dysfunction is acute cardiac impairment caused by sepsis, characterized by reduced contractility, possible biventricular dilation, low ejection fraction, and poor response to fluids, often occurring with circulating inflammatory mediators and mitochondrial dysfunction.
Isoflurane in the PICU
Q: Monica, let's start with a basic question.
Isoflurane is chosen for its favorable balance of potency, lower fluoride release compared with sevoflurane, and practical considerations like MAC and blood-gas properties, making it suitable for refractory asthma and other PICU scenarios when inhaled anesthesia is needed.
Von Willebrand Disease in the PICU
Q: When we are seeing a patient that may be having a lot of bleeding, how do we make the diagnosis of Von Willebrand Factor Disease?
Pradip discusses that a history of bleeding, especially mucosal bleeding or early bruising, should alert clinicians and that definitive tests are needed to diagnose Von Willebrand disease.
