The most promising scenarios are in patients who are surgical candidates undergoing standard care: vaccines given after surgery to prevent recurrence, vaccines in the neoadjuvant/adjuvant setting to tackle microscopic disease, and preventive vaccination in at-risk individuals to establish immune memory before cancer develops. In each case, coupling the vaccine with an antibody like anti-PD-1 and possibly a stimulator such as anti-CD137 appears to enhance effectiveness.

Q: So what's the problem with immunotherapy in pancreatic cancer compared to other cancers?
The main issue is that pancreatic cancers typically have far fewer mutations, so there are fewer neoantigens for immune cells to recognize. Immune modulators can release the brake on immune cells, but without enough tumor-specific signals, the immune response remains weak. Vaccines targeting tumor antigens can prime T cells to recognize the tumor, and combining these vaccines with immune modulators helps overcome the tumor's immune evasion.
