  • Fantastic Show!

This show is amazing! As someone who has been dealing with complex chronic illnesses for almost 10 years, I have found this to be one of the most informative and empathetic podcasts I have listened to on the subject matter. I adore Emily Kate Stephens, and I think the addition of Gez Medinger’s expertise has been wonderful.

I would love if y’all interviewed Dr. Andrea Chadwick of Swing Care and Dr. Ginevra Liptan, who wrote the Fibro Manual. (Swing Care is an amazing telehealth program for peo... more

  • Very helpful

I’ve found this podcast very helpful for myself as I navigate Long COVID and ME/CFS. Would love an episode specifically on nutrition, especially as it pertains to mitochondrial health. An episode on caregiving while sick (for kids or elderly) would also be great. Thank you!

  • Now it’s excellent

I am updating my early criticism, now the show is excellent! The sleep one was so helpful, and think the interplay between the scientific speaker and how you have both implemented stuff is the way to go, really relatable and informative. Not to mention so sick of listening to other podcasts on sleep trot out the same ole same ole for healthy ppl who simply don’t prioritise sleep! One thing though you ask for feedback but there is no email address so how? Will you even read this? And lastly promp... more

Talking Points

Recent interactions between the hosts and their guests.

#32 Hidden Virus, Immune Exhaustion & the Brain: Long Covid, ME/CFS and post-viral illness with Dr Avindra Nath (NIH)
Q: What are the potential therapeutic avenues discussed, and what are the caveats?
Therapies like IVIG and PD-1 checkpoint inhibitors are being explored to reset immune function, but there are safety concerns and the need for rigorous trials to avoid autoimmunity or unintended complications before they can be recommended.
#32 Hidden Virus, Immune Exhaustion & the Brain: Long Covid, ME/CFS and post-viral illness with Dr Avindra Nath (NIH)
Q: What did your deep phenotyping study reveal about immune differences between men and women with ME/CFS?
The study found divergent immune activation patterns: more B cell activation in men and more T cell activation in women, with both groups showing immune exhaustion, suggesting sex-specific pathways in disease mechanisms and implications for treatment.
#32 Hidden Virus, Immune Exhaustion & the Brain: Long Covid, ME/CFS and post-viral illness with Dr Avindra Nath (NIH)
Q: Can you summarize how a virus can impact brain function even if only a few cells are infected?
Even when only a small number of brain cells are infected, viral proteins and activated glial cells can release substances like cytokines that spread dysfunction across larger networks, leading to widespread neuroinflammation and symptoms.
#31 POTS: Symptoms, understanding, and management with Dr Tae Chung
Q: What are the main treatment approaches for POTS?
Treatment focuses on volume expansion (increasing blood volume via fluids, salt intake, and sometimes medications like midodrine or pyridostigmine), along with targeted medications to manage tachycardia (beta blockers, ivabradine) and a supervised exercise program that is integrated with medical therapy. Non-pharmacological strategies and wearable monitoring also play a key role in guiding therapy.
#31 POTS: Symptoms, understanding, and management with Dr Tae Chung
Q: How is POTS diagnosed in clinical practice and what tests are used?
Clinicians use a combination of history, physical exam, and tests such as the active stand/NASA lean test to measure heart rate changes on standing. A positive result typically shows the heart rate increasing by more than 30 bpm within 10 minutes (40 bpm in children) with possible blood pressure changes and symptom provocation, while ensuring no underlying heart structure disease explains the symptoms.
