  • Great learning on my commute.

I'm absolutely loving all the content! With a long commute these days, this podcast has been a fantastic companion—I’ve enjoyed nearly every episode, even those that don’t directly relate to my current ENT practice. I’ve never done endoscopic ear surgery, but after hearing the episode with Brandon Isaacson, I’m now eager to find a course or check out some of his Instagram videos. I really wish this podcast had been around during my residency. It’s incredibly well done, and the production qualit... more

Talking Points

Recent interactions between the hosts and their guests.

Ep. 276 LPR vs. LPS: Key Differences & Diagnostic Techniques with Dr. Inna Husain
Q: What is your preferred diagnostic approach when LPRD is suspected but endoscopy is normal?
Start with a thorough history and targeted laryngoscopic evaluation, then proceed with 24-hour pH impedance testing to capture both acid and non-acid reflux and to correlate with symptoms, rather than relying on BRAVO alone.
Ep. 274 Pediatric Thyroid Nodules: Clinical Insights & Approaches with Dr. Wen Jiang
Q: What factors influence the choice between lobectomy and total thyroidectomy in children with suspected or confirmed cancer?
Low-suspicion lesions may be managed with lobectomy, while high-suspicion mutations or extensive disease often lead to upfront total thyroidectomy; surgical planning prioritizes removing disease while avoiding excessive risk, such as unnecessary central neck dissection.
Ep. 274 Pediatric Thyroid Nodules: Clinical Insights & Approaches with Dr. Wen Jiang
Q: How does the team approach workup and decision-making for a pediatric patient with a Bethesda 3 or 4 nodule?
We use a collaborative approach with endocrinology, pathology, and IR to perform ultrasound-guided FNA with rapid adequacy testing, followed by DNA-based molecular testing to refine surgical planning and reduce the need for diagnostic lobectomy when possible.
Ep. 274 Pediatric Thyroid Nodules: Clinical Insights & Approaches with Dr. Wen Jiang
Q: What are the key differences between pediatric and adult thyroid nodules in terms of incidence and malignancy risk?
Pediatric nodules are less common overall but carry a higher malignancy risk; teenagers show genetic/molecular profiles similar to young adults, while younger children tend to have different mutations and clinical behavior, often presenting with lymph node involvement yet with excellent outcomes.
Ep. 272 Allergy Education on Social Media and Beyond: Challenges and Strategies with Dr. Zachary Rubin
Q: What advice would you give to other physicians starting a public voice?
Choose one platform to start, play to your strengths (writing vs. video), maintain authentic branding, and build a routine for consistent content creation; also understand medical boundaries and engage responsibly with followers.
