  • Great learning on my commute.

I'm absolutely loving all the content! With a long commute these days, this podcast has been a fantastic companion—I’ve enjoyed nearly every episode, even those that don’t directly relate to my current ENT practice. I’ve never done endoscopic ear surgery, but after hearing the episode with Brandon Isaacson, I’m now eager to find a course or check out some of his Instagram videos. I really wish this podcast had been around during my residency. It’s incredibly well done, and the production qualit... more

Talking Points

Recent interactions between the hosts and their guests.

Ep. 269 Management of Sinonasal Tumors with Skull Base Involvement: Part 1 with Dr. Zara Patel, Dr. JP Almeida, & Dr. Garret Choby
Q: How do you decide between an endoscopic approach and an external approach like Denker's maxillectomy or prelacrimal access?
The decision hinges on the location and extent of bony attachments, especially anterior maxillary wall involvement and exposure needs around the orbit and skull base. If anterior or medial attachments are extensive or poorly accessible endonasally, an external approach such as Denker's or prelacrimal access may be planned to ensure complete removal and margins while preserving function where possible.
Ep. 269 Management of Sinonasal Tumors with Skull Base Involvement: Part 1 with Dr. Zara Patel, Dr. JP Almeida, & Dr. Garret Choby
Q: What are the initial steps you take when approaching a sinonasal tumor patient with potential skull base involvement?
We start with confirming whether we have a preoperative diagnosis or whether an office biopsy is needed, assess whether the lesion is confined to the nasal cavity or extends to sinuses and skull base, and plan imaging (MRI and CT) accordingly to map attachments and potential invasion before deciding on endoscopic versus external approaches.
Ep. 267 Clinical Hypnotherapy Applications in ENT Medicine with Dr. Sonia Gupte
Q: What is hypnotherapy, and how does it differ from hypnosis in practice?
Hypnotherapy involves guiding the patient through an altered state to uncover the why behind symptoms and then recode the mind, whereas hypnosis is a one-way process focused on relaxation and guided imagery without the same active cognitive reprocessing.
Ep. 267 Clinical Hypnotherapy Applications in ENT Medicine with Dr. Sonia Gupte
Q: Can you first tell us a little bit about yourself and how you got into hypnotherapy?
Dr. Sonia Gupte describes shifting from traditional medicine to mind-focused therapies after personal family experiences, attending a weekend hypnotherapy class that dramatically changed her approach and led to a career in clinical hypnotherapy and education.
Ep. 263 Immunotherapy for Adults with RRP with Dr. Aaron Friedman
Q: When do you start considering adjuvant therapies like bevacizumab or Sadofovir, and how do you integrate them into surgical management?
The guest describes using intralesional bevacizumab early in treatment due to its favorable safety profile and potential to extend intervals between surgeries, while Sadofovir is more controversial due to mixed data on scar risk and malignant potential.
