Talking Points

Recent interactions between the hosts and their guests.

Targeting the Lectin Pathway in Hematopoietic Stem Cell Transplant-Associated Thrombotic Microangiopathy
Q: Would you consider using NASPRIMA as a first-line therapy if available, and how would you approach duration and monitoring?
Yes, strongly consider NASPRIMA for high-risk TATMA when available, particularly given poor outcomes with standard care. Start early, monitor via clinical and laboratory trends, keep a four-to-eight-week framework as a starting point for duration, and tailor the plan based on recovery of organ function and reduction in LDH and transfusion needs.
Targeting the Lectin Pathway in Hematopoietic Stem Cell Transplant-Associated Thrombotic Microangiopathy
Q: How did you address the challenge of combining data from trials, expanded-access programs, and registries?
We harmonized definitions and focused on high-risk TATMA features, using inverse probability of treatment weighting to balance prognostic factors across diverse data sources. Despite differences in how patients were treated and diagnosed in each source, the external control design aimed to approximate a randomized comparison and showed consistent survival benefit.
Targeting the Lectin Pathway in Hematopoietic Stem Cell Transplant-Associated Thrombotic Microangiopathy
Q: Can you summarize the rationale, design and major findings of your study?
The rationale stems from the urgent need to treat a high-mortality complication after transplant; the initial single-arm trial showed encouraging response and survival, so we built an external-control analysis using three data sources to compare outcomes and adjust for baseline differences. The main finding was that NASPRIMA treatment was associated with a substantial survival benefit, with hazard ratios around 0.25–0.38 across cohorts, supporting a real-world survival advantage.
Search for the Fountain of Youth: Can Bispecific Antibodies Resurrect Dysfunctional T Cells to Improve Outcomes in Richter Transformation?
Q: How does CRS influence treatment decisions, and what dosing strategies were used to mitigate it?
CRS remains a significant consideration; employing a three-dose-step up dosing scheme for Acrytomab significantly reduced CRS severity and improved tolerability, particularly important in Richter transformation where T-cell function is compromised.
Search for the Fountain of Youth: Can Bispecific Antibodies Resurrect Dysfunctional T Cells to Improve Outcomes in Richter Transformation?
Q: What was the primary endpoint for the cohort and what were the key findings?
The primary endpoint was overall response rate, with secondary endpoints including duration of response, progression-free survival, and overall survival. In the cohort, first-line patients showed a higher overall response rate and a greater proportion achieving complete remission compared to those treated in later lines, with durable responses observed in the first-line group.
