Q: How do you view the role of MRD testing in transplant decisions and maintenance strategies?
MRD testing is informative but not the sole determinant for transplant timing; fixed induction followed by transplant remains a common approach, and decisions about MRD-directed transplant are still evolving. In maintenance, MRD status contributes to tailoring whether doublet maintenance or single-agent maintenance is appropriate, but duration and cost considerations are also critical.

Q: What are your thoughts on when to perform a kidney biopsy or how to handle suspected myeloma cast nephropathy in the setting of high creatinine and high free light chains?
In suspected cast nephropathy, you should pursue rapid disease control with effective anti-myeloma therapy and consider plasma exchange where available, especially with very high light chains, though some settings can manage without exchange if the patient rapidly improves on therapy. Kidney biopsy is reserved for unclear cases where alternative etiologies are suspected or when light-chain burden is not clearly driving the injury.

Q: Vincent, how do you approach a newly diagnosed patient in clinic, and how does risk stratification influence your induction choice?
For most newly diagnosed patients, the initial approach is to consider a quadruplet induction (Dara-VRd or Isa-VRd) if the patient can tolerate it; otherwise a triplet (Dara-Rd or Isa-Rd) may be used with dose adjustments. Risk stratification informs maintenance strategy and transplant planning more than it changes the induction itself in many cases, with frailty and performance status guiding how aggressive the upfront therapy is.
