    Good Topics and Content.

“Keys to the Cart” difficult to listen to. Podcast speakers sound stiff and non-fluent. It sounds like a non-medical person reading content off cue cards or out of a medical textbook.

Talking Points

Recent interactions between the hosts and their guests.

Article of the Month – September 2025 – Satya Krishna Ramachandran and Robert Farney
Q: What might it take to integrate this kind of detection of ataxic breathing into mainstream monitoring tools for OIRD?
A standard definition and improved training for clinicians on recognizing ataxic breathing patterns would be critical, along with technology integration into monitoring systems.
Article of the Month – September 2025 – Satya Krishna Ramachandran and Robert Farney
Q: What does this tell us about the current limitations of how we detect opioid toxicity?
Many patients can have opioid toxicity and appear mentally alert, making traditional monitoring metrics like oxygen saturation and respiratory rate unreliable.
Article of the Month – September 2025 – Satya Krishna Ramachandran and Robert Farney
Q: Could you explain why you chose these two medications?
Remifentanil is highly potent and ultra short-acting, making it ideal for inducing OIRD in a controlled setting, while low-dose propofol simulates mild sedation, which is crucial since opioid toxicity increases in such states.
Article of the Month – September 2025 – Satya Krishna Ramachandran and Robert Farney
Q: Can you explain what ataxic breathing is and how it differs from more commonly monitored signs of respiratory depression like low respiratory rate?
Ataxic breathing is characterized by irregular inter-breath intervals and variable tidal volumes, unlike the more easily measurable, but often unreliable respiratory rate.
Article of the Month – July 2025 – Thomas Hemmerling and Arman Valadkhani
Q: What was your primary outcome of the study?
The primary outcome was defined as a perioperative rise in high-sensitive troponin T, measured up to 48 hours post-operatively.
