  • Oncology Brothers: Setting a New Standard in Medical Education

The Oncology Brothers have become a go-to in my week. They take the constant firehose of oncology data—new FDA approvals, shifting algorithms, and “practice-changing” abstracts—and turn it into something practical I can actually use in clinic the next day. The conversations feel like sitting down with smart colleagues who care about evidence, clarity, and real-world decision making, not just reciting trial slides. It’s exactly the kind of thoughtful, high-yield education our field needs more of.... more

Thank you so much for putting this together! Love the updates of ongoing research! I am the primary caretaker for my 79 year old dad who was diagnosed 6/24 with stage IV CRC MSS KRAS G13D TMB-H, so love your GI updates from ASCO ‘25 and subcutaneous nivo, which we are considering. Hope you do more like bot/bal and neodadjuvant single cycle pembro. Thanks for taking the time to do this!!

Talking Points

Recent interactions between the hosts and their guests.

Treatment Algorithm of Late Relapsed/Refractory Multiple Myeloma (RRMM)
Q: But what next? What should we do for refractory disease after second, third, fourth line?
The next steps depend heavily on antigen expression after prior therapies; if BCMA remains expressed, BCMA-targeted strategies remain viable, but if BCMA is lost, we pivot to alternative targets such as GPRC5D or non-BCMA regimens, with careful consideration of prior toxicities and lines of therapy.
Treatment Algorithm of Early Relapsed/Refractory Multiple Myeloma (RRMM)
Q: How are you sequencing bispecific antibodies, CAR T, for standard-risk or elderly patients after first relapse?
The panel generally prioritizes CAR-T first when feasible due to higher response durability, followed by bispecific antibodies if CAR-T isn't suitable; in older or less-healthy patients, bispecifics or conventional triplets become more prominent options, with careful consideration of toxicity and logistics.
ASCO 2026 Breast Highlights: OPTIMA, lidERA, VIKTORIA-1, KEYNOTE-522, ASCENT-03/04, TROPION-Breast02
Q: Can you touch on the Ascent-04/Ascent-03 updates and what they mean for sequencing and choosing Between Sacituzumab Govitecan and Dato-DXD in PD-L1–positive and PD-L1–negative TNBC?
Both regimens improved progression-free survival, with SG plus pembrolizumab showing strong activity in PD-L1–positive disease and Dato-DXD offering a different toxicity profile; decision-making will depend on patient tolerability, schedule, and the ability to manage specific adverse events such as mucositis or eye toxicity, as well as subsequent lines of therapy.
ASCO 2026 Breast Highlights: OPTIMA, lidERA, VIKTORIA-1, KEYNOTE-522, ASCENT-03/04, TROPION-Breast02
Q: Hope, to confirm what you touched on, for that 40-year-old premenopausal node-positive patient, are we moving toward testing like ROR to guide adjuvant decisions and possibly spare chemotherapy?
Yes, the panel is adopting Prosigna-based or similar risk scores to tailor adjuvant therapy, with ovarian suppression still important, and there is growing comfort using the risk of recurrence score to steer decisions, potentially reducing unnecessary chemotherapy in premenopausal patients.
Challenging Cases in Diffuse Large B-cell Lymphoma (DLBCL) – Drs. Gilles Salles & Matt Lunning
Q: What are the key options after early relapse following R-CHOP in young patients?
CAR T-cell therapy has become a central option, with pivotal trials Zuma-7 and TRANSFORM supporting axi-cel and liso-cel use; pre- and post-apheresis strategies and selecting the product based on safety profiles and center capabilities are critical.
