  • Oncology Brothers: Setting a New Standard in Medical Education

The Oncology Brothers have become a go-to in my week. They take the constant firehose of oncology data—new FDA approvals, shifting algorithms, and “practice-changing” abstracts—and turn it into something practical I can actually use in clinic the next day. The conversations feel like sitting down with smart colleagues who care about evidence, clarity, and real-world decision making, not just reciting trial slides. It’s exactly the kind of thoughtful, high-yield education our field needs more of.... more

Talking Points

Recent interactions between the hosts and their guests.

Challenging Cases in Relapsed/Refractory Acute Myeloid Leukemia (AML): Dr. Joshua Zeidner
Q: What makes Revumenib a preferred second-line option after relapse post-transplant in KMT2A rearranged AML?
Revumenib demonstrates higher response rates (around 60%+ overall) and meaningful remissions, making it a strong bridging therapy to longer-term strategies like transplant, with tolerability that supports outpatient or less intensive management.
Managing Toxicities of BRAF-MEK Inhibitors in Non-Small Cell Lung Cancer – Dr. Melissa Johnson
Q: What are the two main BRAF-MEK inhibitor combinations discussed, and how do their efficacy data compare?
The two combinations are Dabrafenib plus Trametinib, with earlier data showing meaningful responses and longer OS in melanoma and NSCLC, and Encorafenib plus Binimetinib, which recent data demonstrate longer progression-free survival and favorable overall survival in NSCLC; the choice depends on tolerability and patient-specific considerations.
How to Treat Metastatic NSCLC W/O Targeted Mutations – Treatment Algorithm with Dr. Christine Garcia
Q: What are the key considerations when choosing between Adagrasib and Sotorasib for KRAS G12C?
Choose based on CNS penetration, brain metastases status, drug interactions, QT prolongation risk, and patient-specific comorbidities; CNS involvement often steers toward Adagrasib, while overall tolerability and prior therapies also shape the choice.
How to Treat Metastatic NSCLC W/O Targeted Mutations – Treatment Algorithm with Dr. Christine Garcia
Q: When is chemotherapy added to immunotherapy in high PD-L1 disease?
Chemotherapy is considered for rapid disease control in patients with high disease burden, symptomatic disease, younger patients, or those with extensive liver metastases, where waiting 8-12 weeks for IO effects is not feasible.
How to Treat Metastatic NSCLC W/O Targeted Mutations – Treatment Algorithm with Dr. Christine Garcia
Q: How do you decide between single-agent immunotherapy and dual checkpoint inhibition in high PD-L1 Expressors?
I consider the whole molecular context and biomarkers beyond PD-L1, especially STK11/KEAP1 co-mutations, TMB, and overall disease biology; dual IO is reserved for a small, carefully selected subset rather than used universally.
