  • Oncology Brothers: Setting a New Standard in Medical Education

The Oncology Brothers have become a go-to in my week. They take the constant firehose of oncology data—new FDA approvals, shifting algorithms, and “practice-changing” abstracts—and turn it into something practical I can actually use in clinic the next day. The conversations feel like sitting down with smart colleagues who care about evidence, clarity, and real-world decision making, not just reciting trial slides. It’s exactly the kind of thoughtful, high-yield education our field needs more of.... more

Thank you so much for putting this together! Love the updates of ongoing research! I am the primary caretaker for my 79 year old dad who was diagnosed 6/24 with stage IV CRC MSS KRAS G13D TMB-H, so love your GI updates from ASCO ‘25 and subcutaneous nivo, which we are considering. Hope you do more like bot/bal and neodadjuvant single cycle pembro. Thanks for taking the time to do this!!

Recent interactions between the hosts and their guests.

Clinical Insights: Evolving Landscape of Bladder Cancer After GU ASCO 2026
Q: What is the role of adding IO to BCG in non-muscle-invasive disease, and who should lead this decision in practice?
Data from Crest and Potomac suggest IO addition can benefit high-risk NMIBC, but management requires close collaboration; urologists may lead in many community settings for practical administration, with medical oncologists guiding systemic toxicity management and long-term follow-up.
Clinical Insights: Evolving Landscape of Bladder Cancer After GU ASCO 2026
Q: Tomorrow in your practice, are you going to pick amongst Niagara GEM-Sysderva and EV-Pembro for a Cis-eligible patient population?
Given the data, EV-Pembro is becoming the regimen of choice due to superior path CR and manageable toxicity, but selection will still depend on patient-specific factors like cisplatin eligibility, surgical timing, and potential toxicity; some cases may still consider GEM-Cis in particular contexts.
Combination Approaches in PD-L1–Positive Metastatic Triple Negative Breast Cancer
Q: What did Ascent04 show regarding sacituzumab govotecan with pembrolizumab versus standard chemo with pembrolizumab?
Ascent04 demonstrated superiority in progression-free survival for the sacituzumab and pembrolizumab arm compared with the standard keynote chemotherapy arm, with longer duration of response and an overall survival benefit that is still being clarified as data mature.
Combination Approaches in PD-L1–Positive Metastatic Triple Negative Breast Cancer
Q: How do you define PD-L1 positivity and what assays are available for metastatic TNBC?
PD-L1 positivity in this setting primarily uses CPS, a combined positive score that considers PD-L1 staining in immune cells; a CPS of 10 or more is the current threshold for pembrolizumab in this context, though testing can vary by lab and pathologist, so binary 10-plus versus less-than-10 is often preferred.
Triple Negative Breast Cancer (TNBC) Treatment Algorithm: Dr. Tiffany Traina
Q: What is your approach to residual disease after adjuvant chemo-immunotherapy in the TNBC setting?
Any amount of invasive residual disease warrants additional systemic therapy, with CAPE-Cytobine in a one-week-on/one-week-off schedule or olaparib for BRCA-mutant cases, depending on biomarkers and prior treatments.
