Talking Points

Recent interactions between the hosts and their guests.

S17 Ep70: Navigating Fixed-Durations Regimens in CLL and Improving Outcomes for High-Risk Patients: With John N. Allan, MD
Q: In your review, what is one short take-home message for colleagues about this work?
Reviews synthesize a broad and rapidly evolving evidence base, providing a consolidated, up-to-date reference that helps clinicians quickly locate data, apply current best practices, and anticipate future directions, including the role of MRD guidance and fixed-duration strategies in everyday patient care.
S17 Ep70: Navigating Fixed-Durations Regimens in CLL and Improving Outcomes for High-Risk Patients: With John N. Allan, MD
Q: Following the review, what are the biggest remaining unmet needs in CLL regarding fixed-duration regimens?
Key unmet needs include optimizing treatment for high-risk patients such as those with 17p deletions or complex karyotypes, understanding long-term outcomes and resistance patterns, and integrating MRD-guided strategies to tailor duration and intensity of therapy, as well as exploring new molecules and combinations to address resistance mechanisms.
S17 Ep70: Navigating Fixed-Durations Regimens in CLL and Improving Outcomes for High-Risk Patients: With John N. Allan, MD
Q: In your review, what was the most striking finding regarding safety among fixed-duration regimens in CLL?
Second-generation BTK inhibitor–venetoclax combinations showed favorable cardiovascular safety, with very low atrial fibrillation and cardiovascular event rates in frontline studies, while cytopenias and infections were generally higher with triplet regimens but manageable with careful monitoring, indicating a favorable safety landscape for fixed-duration regimens overall.
S17 Ep70: Navigating Fixed-Durations Regimens in CLL and Improving Outcomes for High-Risk Patients: With John N. Allan, MD
Q: Why are fixed-duration regimens so important?
Fixed-duration regimens offer a balance between effective disease control and reduced long-term exposure to drugs, potentially lowering cumulative toxicity. They provide a defined treatment window, allow patients to be off therapy earlier, and can simplify management while maintaining comparable efficacy in many patients, especially with newer combinations showing favorable safety signals.
S17 Ep70: Navigating Fixed-Durations Regimens in CLL and Improving Outcomes for High-Risk Patients: With John N. Allan, MD
Q: Can you give us background on what the treatment landscape looks like for CLL right now? How does this current landscape compare to the past?
The landscape has shifted toward fixed-duration oral regimens, particularly BTK inhibitors plus venetoclax, and new fixed-duration approaches including calabrutinib-venetoclax. While continuous therapies remain an option, fixed-duration regimens are increasingly practical, with efforts to personalize therapy based on patient risk and response, and ongoing comparisons to older regimens indicate similar efficacy with potentially better safety and stop-ability for many patients.
