We need to include diverse voices (social scientists, economists, and community representatives) at the table when guidelines are developed, invest in training new generations of clinicians and educators to foreground health equity, and embed policies that discourage race-based proxies in clinical decision-making.

The problem became obvious to me during my early medical training when I heard peers and mentors use demeaning language about patients from marginalized communities, which made me aware that bias was normalized in care delivery and affected treatment decisions.

He expressed concern about the approach but noted the fierce push from advocates and the goal of helping Black patients, while acknowledging the misalignment between clinical benefit and the marketing narrative.

The drug showed benefits in Black patients, but the marketing and pricing strategy, including insurance substitution with two cheaper generics, undermined its viability and suggested deeper issues with race-based indications.
