  • Tackles tough questions

Great show to listen to for everyone, especially if you are in healthcare. The recent episode about the criminality of Johnson and Johnson was superb and the discussion of 340b, the poor behavior of the AMA and many other great topics were wonderful to listen it. Chadi tackles the maligned incentives of healthcare companies, hospitals, and clinicians while also giving great updates across many fields of medicine.

Talking Points

Recent interactions between the hosts and their guests.

Episode 282: TIL Therapy and Melanoma With Dan Olson
Q: What makes the manufacturing timeline so critical for TIL therapy?
The process includes surgical tumor extraction, lab expansion over several weeks, quality testing, and cryopreservation; this means a substantial waiting period before infusion, which impacts patient selection, bridging therapies, and overall treatment planning.
Episode 282: TIL Therapy and Melanoma With Dan Olson
Q: Can you simplify TIL Therapy for someone new to it?
TIL Therapy starts by taking a patient's own tumor-infiltrating T cells, expanding them in the lab to large numbers, and then infusing them back after a lymphodepleting preconditioning regimen, followed by high-dose IL-2 to support T cell activity; the approach relies on the patient's own immune cells to target melanoma antigens, and outcomes are most favorable when used earlier in the disease course.
Episode 283 - MAID in America: Navigating Choice, Compassion, and Controversy
Q: Chandana, what exactly is MAID and how does it differ from euthanasia or suicide?
MAID involves a patient self-administering prescribed medications to die, in contrast to euthanasia where a clinician administers the medications; MAID is used for terminal illnesses with the patient's voluntary consent and under strict safeguards.
Episode 281: Precision Oncology in India With Sewanti Limaye
Q: Would you do it all over again, leave the US for India, and what keeps you motivated?
Yes, the move is challenging but meaningful; the host maintains ties to the US through private concierge services while focusing on serving the Indian population. The work is life-fulfilling, especially in building trial access, advocacy, and early-diagnosis initiatives, despite the personal and logistical challenges of cross-continental work.
Episode 281: Precision Oncology in India With Sewanti Limaye
Q: Which trials can be executed in India given resources, and what is the landscape for early versus late phase studies?
Late-phase trials are more widely feasible, while early-phase (especially Phase 1) remains a work in progress in India. Phase 1Bs with dose expansion are more practicable, and leading centers like the host institution and other major hospitals are contributing to the growth of early trials, with broader global collaboration and private sector involvement shaping the evolution.
