  • Tackles tough questions

Great show to listen to for everyone, especially if you are in healthcare. The recent episode about the criminality of Johnson and Johnson was superb and the discussion of 340b, the poor behavior of the AMA and many other great topics were wonderful to listen it. Chadi tackles the maligned incentives of healthcare companies, hospitals, and clinicians while also giving great updates across many fields of medicine.

Talking Points

Recent interactions between the hosts and their guests.

Episode 277 - When “Yes” Becomes Too Much: Confronting Physician Burnout
Q: What made you decide to share your burnout story publicly?
I hoped that sharing would help others by demystifying the recovery process, encouraging disclosure, and guiding people to resources like intensive outpatient programs; it was also about validating that physicians can seek help without stigma and that leadership and colleagues can support sustainable changes.
Episode 277 - When “Yes” Becomes Too Much: Confronting Physician Burnout
Q: Let me start by asking you, how would you define burnout to people who are listening or viewing this?
Burnout is a sustained sense of overwhelm and weariness where there's more to do than time to do it, often overlapping with anxiety or depression, but not always equating to them; it's a condition that can persist even outside the clinical definitions and is tied to workload, personal commitments, and the environment.
Episode 276: Radiation Therapy for GI Cancer With Nina Sanford
Q: What are the practical considerations when choosing proton therapy for GI cancers?
Protons can spare normal tissue by limiting exit dose, but their value depends on the specific anatomy and goals; decisions should weigh data (trial design and endpoints), practicality (travel and cost), and whether conventional methods already achieve adequate sparing plus tumor control.
Episode 276: Radiation Therapy for GI Cancer With Nina Sanford
Q: How is adaptive radiation envisioned to impact GI radiotherapy in the coming years?
Adaptive radiation aims to modify the treatment plan as the tumor and surrounding tissues change during therapy, potentially increasing organ preservation and reducing toxicity by achieving the right balance of dose.
Episode 276: Radiation Therapy for GI Cancer With Nina Sanford
Q: What has changed in upper GI cancers regarding the role of radiotherapy with trials like ESOPEC and Matterhorn guiding care?
Radiation is no longer universally used across all upper GI cancers; instead, decisions hinge on histology, tumor location, and integration with systemic therapy. For adenocarcinomas at the GE junction, perioperative chemotherapy is often favored, while squamous cell cancers may still benefit from preoperative chemoradiation with added immunotherapy in some settings.
