Q: What would be your first line antihypertensive for someone with palpitations and hypertension?
I would generally try to just lower the blood pressure, as beta blockers aren't necessarily very good antihypertenses and aren't the best tolerated medication.

Q: If someone comes to you and they don't have any prior cardiac history, their history seems fairly reassuring and there's none of those red flags. Are you fairly happy that the investigations end there and that we can reassure our patients that their palpitations are benign?
It depends on the patient's perspective, but generally if there's a reassuring history and ECG, reassurance can often be provided.

Q: What do you do? Could you talk us through your physical exam?
The examination is usually normal, but it's important to document their heart rate, regularity, and blood pressure. Abnormal findings should be carefully documented.

Q: Is there anything within the history with the patients telling us that sort of picks your ears up and you think, oh, this sounds a bit more like it could be primarily a cardiac problem?
People with a known diagnosis of underlying heart disease, especially those with a history of ischemic heart disease or cardiomyopathy, should raise suspicion for an underlying concerning arrhythmia.

Q: Could you talk us through your approach to the patient who presents with palpitations?
A degree of active reassurance is really quite important, and oftentimes the treatment for this is the consultation. Most of the time, palpitations are not dangerous and are usually benign.
