Connecting Hematology Research and Real Life: Peter Kapitein
Q: To start off, Peter, you often say that advocacy means connecting people who should already be connected. What inspired you to take on that mission?
Researchers, doctors, and industry already meet at events, but true progress comes from deliberately connecting the right people who can move the needle for patients, not just those who normally cross paths.
Menin Inhibitors to Treat Acute Myeloid Leukemia (AML)
Q: What is the safety profile and what resistance mechanisms are being observed with these drugs?
The safety profile is generally manageable but includes differentiation syndrome and QT prolongation, with some drug-specific differences between agents. A key resistance mechanism is mutations in the menin binding pocket that prevent drug binding, occurring in a substantial subset of patients at progression, which motivates ongoing development and combination strategies.
Menin Inhibitors to Treat Acute Myeloid Leukemia (AML)
Q: Why are menin inhibitors a therapeutic option for AML, and which patients are most likely to benefit?
Menin inhibitors disrupt the menin-KMT2A axis, inactivating oncoproteins that drive leukemogenesis in specific AML subtypes, especially MLL/KMT2A-rearranged and NPM1-mutant AML. These subtypes show meaningful response rates in trials, and these responses have led to regulatory approvals in relevant relapse/refractory settings.
How Experts Treat HIV-associated Lymphomas
Q: How were these guidelines developed across different European countries?
A core group of experienced investigators from multiple European countries collaborated over more than two years to reach consensus, addressing variations in treatment practices and aiming for a unified set of recommendations.
How Experts Treat HIV-associated Lymphomas
Q: Why is it important to have specific practical guidelines on HIV lymphomas?
Guidelines are needed because HIV-associated lymphomas have different outcomes and require tailored treatment approaches; ART should not be stopped during lymphoma therapy, and there are specific considerations for therapies like rituximab and CAR-T cells depending on immune status.
