    I love this podcast! I am a first year I see you attending, and I have the privilege of working in a very busy hospital with surgical, medical, transplant, cardiology, and a new ICU. This podcast really reinforces concepts and cutting edge things in our field. the only gripe i have is that the volume levels are all over the place!     I’m a critical care fellow in South Africa and am currently preparing for my final exam coming up in 4 months. I’ve only recently discovered this podcast after struggling through a few others.

    I found Critical Matters easy to listen to, highly relevant and current. It feels like one of our Journal Clubs combined with an ISICEM-like feel to the high standard of expertise being shared.

Empiric therapy is started when there is high suspicion of candidemia, particularly in patients with risk factors and sepsis where bacterial infection is not clearly ruled out. Echinocandines are the standard initial agents; duration depends on source control and whether the infection has disseminated, with shorter courses possible after adequate source control, especially for line infections without other foci.

Q: Excellent. I wanted to touch on two aspects of the diagnosis and ask you if you could go a little bit deeper. Could you discuss the two categories of invasive candidiasis and how clinicians approach diagnosis in the ICU at a high level?
Two main forms are candidemia (bloodstream infection) and invasive intra-abdominal candidiasis. Diagnosis is challenging because symptoms mimic bacterial sepsis, and routine blood cultures are used though sensitivity is moderate; adjunct tests and risk-factor-based reasoning help decide on empiric therapy while awaiting results.
