  • Great discussions for all ICU team members!

I love this podcast! I am a first year I see you attending, and I have the privilege of working in a very busy hospital with surgical, medical, transplant, cardiology, and a new ICU. This podcast really reinforces concepts and cutting edge things in our field. the only gripe i have is that the volume levels are all over the place! • Best podcast on Critical Care related matters

I’m a critical care fellow in South Africa and am currently preparing for my final exam coming up in 4 months. I’ve only recently discovered this podcast after struggling through a few others.

I found Critical Matters easy to listen to, highly relevant and current. It feels like one of our Journal Clubs combined with an ISICEM-like feel to the high standard of expertise being shared.

    Thank you so much!

Acute Pulmonary Embolism: Clinical Guideline Update
Q: What is the role of a Pulmonary Embolism Response Team in patient management?
PERTs assemble multidisciplinary experts to discuss and implement advanced therapies for appropriate patients, helping to shorten hospital stays and optimize rapid, evidence-based decision making for complex PE cases.
Acute Pulmonary Embolism: Clinical Guideline Update
Q: How do the guidelines guide long-term anticoagulation decisions after an acute PE?
DOACs are preferred over vitamin K antagonists for most patients, and duration is based on provoked versus unprovoked risk factors; extended therapy may be indicated for persistent risk factors, with dose reductions considered in the extended phase.
Acute Pulmonary Embolism: Clinical Guideline Update
Q: What is the most common initial diagnostic approach for acute PE in the emergency setting?
CTPA is the go-to imaging test because it is widely available, provides high diagnostic accuracy, and has a favorable overall risk-benefit profile compared with alternatives like VQ scans.
PCT Reassessed
Q: What considerations should intensivists have regarding the application of procalcitonin in practice?
Procalcitonin should be used primarily for discontinuation of antibiotics, not for initial initiation, and protocols should be in place to guide its use.
PCT Reassessed
Q: What are the key findings from clinical trials regarding procalcitonin?
The most significant studies show that procalcitonin is effective in determining when to stop antibiotics, contributing to antibiotic stewardship and reducing unnecessary usage.
