Talking Points

Recent interactions between the hosts and their guests.

205-Good ALS starts with great BLS with David Stanton
Q: What role do EMTs and BLS play in Austere Medicine and how do you balance empowering them with safe practice?
EMTs and BLS providers form the foundation of life-saving care in austere settings. By raising EMT skill levels and empowering them to handle critical tasks (within solid training and oversight), the system preserves patient safety while enabling rapid, scalable care delivery when resources are scarce.
205-Good ALS starts with great BLS with David Stanton
Q: Describe the Medic One concept from Dr. Copeland and why you think it leads the world in out-of-hospital cardiac arrest care.
Medic One trainings empower firefighters to perform advanced cardiac life support in the field, creating an early, effective chain of care that improves outcomes before hospital arrival. The system also emphasizes layperson CPR, continuous education, and clinician-backed autonomy for paramedics to deviate from rigid protocols when patient welfare dictates it.
202-A Life in Medicine with Dr Winston de Mello
Q: So tell us a bit about yourself and what's keeping you busy?
I'm focused on burns care in pre-hospital and critical care settings, work on late-stage burn education through SBLS, and contribute to pelvic pain research and management as part of the Valvol Pain Society, while helping advance burns education within CoROM.
193-Severe Malaria Patient Featuring Zach Andrews
Q: Given the dialysis of limited resources, what guided the choice between quinidine and artesunate for severe malaria in this scenario?
Quinidine was used because it was what was available and approved in that remote setting, but artesunate is indicated as first-line by WHO with better safety and faster parasite clearance; the practitioner acknowledged that imperfect tools beat perfect ones when needed and that IV meds were essential due to absorption issues in severe disease.
199-Snake Bites with Dr Evan Baines
Q: What are the regulatory considerations when administering antivenom in the field or abroad?
Be aware of BBIND or local equivalent approvals for non-native antivenoms, understand the regulatory environment, and prepare with authorization from governing bodies to deploy antivenoms under appropriate exceptions.
